Objective: To analyze and study the influencing factors and the intervention effect of hand hygiene practices of clinicians.Methods: The study was conducted on 120 medical staff in 10 key departments of a top three hospital.The survey to investigate the questionnaire method and on-site observation method: the questionnaire for the general situation of medical staff,hand hygiene implementation of the factors that affect the hand hygiene indicators and the mastery of relevant knowledge conducted a survey to understand the hospital's medical staff Hand-hygiene-related occupational literacy;field observation method aims to investigate hand hygiene indications that appear in the unit time period,the implementation rate of hand hygiene indications appear,the pass rate of hand hygiene operations,etc.,hoping to study hand hygiene knowledge and Implementation of the relationship,so as to analyze the relationship between the letter,the line to identify the hand hygiene problems in the hospital to explore measures to enhance the level of hand hygiene.Results: The average score of hand hygiene in this hospital was 21.17 ± 1.39(27 out of 27).Only 27.43% of the respondents reached the hand hygiene knowledge of medical staff in tertiary hospitals.General conditions other than departmental categories had no significant effect on the mastery of hand hygiene(p> 0.05).Of the possible factors that affect the hand hygiene implementation,frequent hand washing damaged hand skin,heavy workload,time-consuming hand hygiene and inadequate(72.5%,55.0%,49.17% respectively)of the medical staffs who received the most attention.The compliance rate of hand hygiene among medical staffs participating in the study was only 30.87%.Among the hand hygiene practices performed above,the passing rate was only 65.9%;there was no obvious effect on the hand hygiene compliance(p> 0.05)except for the departmental category;the hand hygiene related knowledge obtained from the questionnaire Score and hand hygiene compliance rate is not clear(p> 0.05).The author intervenes on the above issues: regular hand hygiene training to strengthen hand hygiene supervision and supervision;improve hand hygiene facilities,to ensure the supply of hand hygiene consumables;to build patient hand hygiene supervision system involved;the above measures are Improve hand hygiene levels have a significant effect(p <0.05).Conclusion: The medical staffs participating in this study have poor mastery of hand hygiene knowledge,poor hand hygiene compliance and low accuracy of hand hygiene implementation.Overall,it is in line with the national average,but it can not meet the requirements of Grade A first-class hospitals.Subjective and objective factors such as lack of awareness of hand hygiene,weak awareness and incomplete hand hygiene facilities have caused the current situation.In view of the above problems,strengthening medical staff hand hygiene knowledge learning and training,strengthening the department hand hygiene facilities update and supply,hospital management departments and departments to strengthen supervision and patient involvement in hand hygiene supervision which is conducive to improving the hospital hand hygiene The situation has a positive effect.The various measures mentioned above go hand in hand.The improvement is not only hand hygiene in the hospital,but also more effective in controlling nosocomial infections and improving the quality of medical care.
