Objective:Through building the experimental hormone osteonecrosis,then observe the c hanges of osteoblasts in the process of building,and traditional Chinese medicine(TCM)lock Yang model under the influence of osteoblast,study traditional Chinese medicine(TCM)lock Yang in a positive role in the treatment of femoral head necrosis,provide r eference basis for new treatment methods.Methods:Experiment 1:By random draw method will be 24 adult healthy New Zealan d white rabbit was divided into three groups,each group of eight New Zealand white r abbit,only one group as normal control group,1 to 2 weeks in model group,model gr oup 4 weeks for another 1 group.Three groups of animals at the same time give regular feed normal feeding,normal control group does not take special processing,and in exp eriment 1 day,use 20 mu g/kg of e.coli endotoxin,via ear marginal vein,injection res pectively in two groups of rabbit model group of animals.After the first 24 hours of e.coli endotoxin injection,namely the experiment 2 days at the same time of the second i njection,injection and drug dose,at the same time use 20 mg/kg coxal muscle injection of methyl prednisolone.After every two days,use the same dose of methyl prednisolon e on rabbit model group 2 groups of gluteal muscle injection.2 set of rabbit model befo re the injection of e.coli endotoxin,a day to give 40000 U/rabbit intramuscular penicill in,prevention of experimental animals injected endotoxin infection occurs,the experimen t began after twice a week for the same amount of intramuscular injection of penicillin.The start day 1,14 days(2 weeks),28 days(4 weeks),to extract femoral arterial blood of three groups of rabbits,observation of arterial blood oxygen partial femoral head using classic enzyme digestion method-the organization method of se paration,extraction and nurture the osteoblast,within the femoral head at the same time to observe each group osteoblast proliferation and extend the rate.All the data collected were analyzed by SPSS20.0 statistical analysis software in the whole experiment process.Results:Experiment 1:The fifth day of the experiment building,2 weeks in model gro up 1 rabbit caused by endotoxin poisoning death,in the experimental building 14 days,four weeks model group,there was a rabbit death due to anesthesia discomfort.Building during 4 weeks by imaging examination,histologic biopsy,femoral arterial blood oxyge n partial pressure detection index changes,confirm building success.1 >At the start,2 and 4 weeks for each group respectively the rabbit bilateral femoral head X-ray films an d MRI examination: normal control group X-ray showed no difference appearance,femor al head shape is normal,uniform bone,bone mineral density is normal,no change,MRI showed no obvious abnormality,joint signal normal;2 Zhou Zao module X-ray showed no obvious change,femoral head form but decreased bone density,t2-weighted MRI ex amination showed a T1,T2 are uneven low signal,a line sample collection,articular ca vity with joint effusion;4 Zhou Zao module X-ray showed femoral head is not smooth and rounded appearance,joint surface is not clear,bone mineral density obviously uneve n distribution,uneven MRI showed low signal area increase,joint effusion.2 >Histologic biopsy: New Zealand rabbit femoral head biopsy showed normal control group head of the bone trabecular structure arrangement orderly,uniform density,bone cells to normal.Model group 2 weeks New Zealand rabbit femoral head biopsy showed bone trabecular distribution order is a bit messy,bone cells decreased significantly,free bone pit.Mode l group 4 weeks within New Zealand rabbit femoral head biopsy showed head obviously thinning disordered arrangement of trabecular bone,clearance,and bone cell 2 weeks m odel group significantly reduced,empty bone pit volume increased significantly.Femoral a rterial blood oxygen partial pressure test: experiment 1 day,3 groups of New Zealand w hite rabbit femoral arterial blood oxygen partial pressure difference;Within 14 days,2weeks model group and model group 4 weeks between femoral arterial blood oxygen pa rtial pressure difference is not obvious(P > 0.05),compared with the normal control gr oup,model group 2,4 weeks femoral arterial blood oxygen partial pressure obviously d eficiencies(P < 0.05);Experiment 28 days,femoral arterial blood oxygen partial pressur e,normal control group > 2 weeks model group > model group 4 weeks,four weeks a nd femoral arterial blood oxygen partial pressure of model group was obviously lower t han that of normal control group(P < 0.01).Femoral arterial blood oxygen partial pressu re test: experiment 1 day,3 groups of New Zealand white rabbit femoral arterial blood oxygen partial pressure difference;Within 14 days,2 weeks model group and model gro up 4 weeks between femoral arterial blood oxygen partial pressure difference is not obvi ous(P > 0.05),compared with the normal control group,model group 2,4 weeks femo ral arterial blood oxygen partial pressure obviously deficiencies(P < 0.05);Experiment 28 days,femoral arterial blood oxygen partial pressure,normal control group > 2 weeksmodel group > model group 4 weeks,four weeks and femoral arterial blood oxygen partial pressure of model group was obviously lower than that of normal control group(P < 0.01).Experiment 2:Building process and experiment 1 is consistent,which in the process of building one rabbit caused by endotoxin poisoning death.In giving drug inter vention to observe the effect,small dose of lock Yang group one rabbit died after lavag e,5 weeks comfort control group one rabbit unexplained deaths.1 > the lock Yang extr act began to intervene,4 weeks and 8 weeks respectively for each rabbit bilateral femor al head X-ray films and MRI: X-ray showed femoral head no improve appearance,comf ort control group joint surface is not clear,uneven MRI showed low signal area area is not smaller than before,more joint effusion;Small doses of lock Yang group X-ray sh owed improved femoral head form,uneven MRI showed low signal area is shrunk,only a small amount of joint effusion;Large dose X-ray showed femoral head lock Yang gr oup form back to normal,the joint surface is clear.MRI showed inhomogeneous low si gnal area basic repair,before the joint effusion disappear.New Zealand rabbit femoral he ad lock Yang group biopsy showed bone trabecular distribution order is a bit messy,bo ne cell populations,trabecular bone gap decreases.High-dose lock Yang group of New Zealand rabbit tissue biopsies showed the avascular necrosis of femoral head control and return to normal levels,trabecular bone structure is clear,structure is normal.3 > femo ral arterial blood oxygen partial pressure detection: use lock Yang after feeding,at the s ame time three groups of the rabbit test showed femoral arterial blood oxygen partial pr essure is from low to high in turn the comfort control group,small dose of lock Yang group,the high-dose lock Yang group,including the lock to Yang intervention in eight weeks,high-dose lock Yang group of rabbit femoral arterial blood oxygen pressure basic returned to normal standard.4 > enzyme digestion method-tissue block method: drug lock Yang before the intervention,the New Zealand white rabbit osteoblast number 3 g roup were lower,no significant difference;Lock to Yang intervention in 4 weeks and 8weeks,the number of osteoblast high-dose lock Yang group > small doses lock Yang co mfort group > the control group,(P < 0.05),there are statistically significant,including experiment at 12 weeks,large doses of the lock Yang group of osteoblast number basic returned to normal levels.Conclusions : In the pathological process of SANFH,there will be a lack of osteoblast,suppressed,ischemia,hypoxia,and so on and so forth,process of necrosis and degree of negative correlation with the number of osteoblast proliferation.Effective ingredients of traditional Chinese medicine(TCM)lock Yang has scavenging free radicals,antioxidation,and enhance immune function,and so on,can be applied to femoral head necrosis of osteoblast and its proliferation effect and lock Yang doses were positively correlated,compared with the control group,the characteristics of existence difference.
