Objective Through randomized and controlled clinical study to evaluate the effectiveness of the method of Jiedu Tongluo on Chinese and western medicine curative effect in treating early diabetic kidney disease of qi and yin deficiency,blood stasis and water stagnation syndrome.Through make a correlation analysis about difference before and after the experiment between the level of p-JNK,NO,TNF-? and 24 h PRO,to explore the possible mechanism and provide the basis for the method of Jiedu Tongluo treatment early diabetic kidney disease.Methods 60 patients with early diabetic kidney disease of qi and yin deficiency,blood stasis and water stagnation syndrome from Inpatient or outpatient department of Endocrinology in the Chinese medicine hospital of Henan province were randomly divided into the method of Jiedu Tongluo treatment group(30 cases)and conventional western medicine control group(30 cases)with a treatment course of 90 days for both groups.Observe the level of 24 h PRO,the level of creatinine,the level of blood sugar,the level of plasma lipid also the level of p-JNK,NO and TNF-? before treatment and after treatment,then make a correlation analysis about difference before and after the experiment between the level of plasma lipid,p-JNK,NO,TNF-? and 24 h PRO.Results 1.Baseline data before treatment in both groups include age,gender,primary disease,duration,type of disease distribution,TCM syndrome score,the level of 24 h PRO,creatinine,blood sugar,plasma lipid,p-JNK,NO and TNF-? there was no statistically significant difference(P>0.05);2.The total effect of clinical efficacy rates in the therapeutic group and control group were 93.3% and 73.3% respectively which with a significant difference(P<0.01);3.After treatment,the total effect of traditional Chinese medicine rates in the therapeutic group and control group were 90.0% and 60.0% respectively which with a significant difference(P<0.05);4.After treatment,the TCM syndrome score and the level of 24 h PRO,p-JNK,NO and TNF-? of the two groups reduced obviously(P<0.01),compared with that of treatment before;After treatment the levels was also obviously lower in the treatment group than that of control group(P<0.05);5.After test the level of blood glucose,creatinine in both treatment group and control group,the differences were not statistical significance(P>0.05);6.After treatment the level of plasma lipid of control group(TC?TG?HDL?LDL)compared with before the experiment has no obvious change,there was no statistically significant difference(P > 0.05);After treatment the level of plasma lipid of treatment groupcompared with before the experiment has no obvious change,there was statistically significant difference(P<0.01);After treatment the levels was also obviously lower in the treatment group than that of control group(P<0.01);7.The level of the 24 h PRO changed before and after the experiment in the control group positively correlated with serum the levels of the p-JNK,NO,TNF-?;8.No obvious side effect was found in the two groups.Conclusion 1.The method of Jiedu Tongluo could improve the Renal function and symptoms in the patients with early diabetic kidney disease of qi and yin deficiency,blood stasis and water stagnation syndrome,The method of Jiedu Tongluo possess satisfactory clinic effect;2.The method of Jiedu Tongluo is benefit in lowering the levels of plasma lipid in patients with early diabetic kidney disease;3.The method of Jiedu Tongluo is benefit in lowering the levels of p-JNK in peripheral blood mono nuclear cells and the level of NO and TNF-?;4.The method of Jiedu Tongluo may play a role of intervention in the development of diabetic kidney disease,through regulate the ROS/JNK pathway.
