The Application Value Of Simple Score System For Small Solid Breast Masses Based On US-BI-RADS

Objective To evaluate the diagnostic efficacy of simple score system in differentiating benign and malignant small breast masses based on US-BI-RADS.Methods1.There were 199 female 217 small breast masses selected as the research objects from January 2013 to December 2015.The age range was from 18 to 72 and the median age was 44.2.Philips HD11、IU22,GE Logiq E9 Color Doppler ultrasonic diagnostic apparatus were used to examination and image acquisition.The default gain was 60%,52%,29%,respectively.The probe frequency was 5-12MHz.3.A retrospective analysis of 217 small solid breast masses(diameter ≤1cm)which ultrasound images were performed.All of the masses were pathologically confirmed.The score items contain irregular shape(1),extremely hypoechogenicity(1),rear echo attenuation(1),microcalcification(1),azimuth unparallel(1),Alder blood flow Ⅱ～Ⅲ(1).4.Calculate the total score of each breast mass and hypothetical fractional boundary of different BI-RADS level.Results1.There were 185 benign masses consisted of 98 zero point,73 one point,12 two point,1 three point,1 four point.The score range was from 0-4.There were 32 malignant masses consisted 1 one point,9 two point,18 three point,3 four point,1 five point.The score range was from 1-5.2.Irregular shape,extremely hypoechogenicity,microcalcification,rear echo attenuation,azimuth unparallel were significantly different between benign and malignant small breast masses(P<0.05).They were regarded as malignant ultrasound features,their positive predictive values(PPV),sensitivity,specificity and accuracy for maligancy were 32.3～66.7%,37.5～96.9%,64.9～96.8%,69.6～88.9%,respectively.Alder blood flow Ⅱ-Ⅲ was no significantly different between benign and malignant small breast masses(P>0.05).3.The area under receiver operating characteristic curve was 0.974.The best cut off was 1.5 point,then the sensitivity and specificity of simple score system in differential diagnosis of benign and malignant small solid breast masses was 96.8%,92.4%,respectively.4.Based on BI-RADS,the total score zero point,one point,two point,three point,>4 point followed by category 3,4a,4b,4c and 5,the positive predictive value was 0.0%,1.4%,42.8%,94.7%,80.0%,respectively.Conclusions The simple score system has a high value in BI-RADS classification of small solid breast masses more objectively.
