ObjectiveThis study aims at the application of mesenchymal stem cells in clinical treatment of a case who was exposed to Iridium-192 source accident in Nanjing,explore its specific repair mechanism.Methods1.Isolated and cultured human umbilical cord mesenchymal stem cells,morphological observation,identification of differentiation ability and detection of surface markers were carried out.2.Observed the chimera starus of hUC-MSCs transplantation by using STR-PCR method.3.Detected the expressions of VEGF in serum and wound tissue with Immunohistochemistry.4.In Nanjing accident,according to the physical dose estimation and biological dose estimation,combined with the clinical symptoms and signs,to definitive diagnosis.5.Pay attention to the whole clinical treatment process of patients,judge the optimal time?delivery method?cell number of application of mesenchymal stem cells and observe the clinical effect.Result1.Combined with radiation dose,auxiliary examination and clinical manifestation,The patient"Wang"who was exposed to“5.7”192Ir source accident in Nanjing got diagnosed mild bone marrow form of acute radiation sickness with?°acute radiation-induced skin injury.2.In the early course of patients,we used stimulation of hematopoiesis,improve microcirculation,nutrition support,prevention of infection and other clinical treatment.The acute bone marrow type into the recovery period 40 days after exposure,However,the patient with local exposure to large dose,wound healing delayed,in order to promote the wound repair,we carried out the intravenous infusion of mesenchymal stem cells after the second flap transplantation,finally the radiation-induced skin injury with surgical healing gradually.3.The results of STR-PCR showed that hUC-MSCs implants was no detected;ELISA showed the expressions of VEGF in serum and wound tissue were significantly higher after hUC-MSCs injection.ConclusionIn Nanjing radiation accident,we successfully isolated umbilical cord mesenchymal stem cells,which were applied to the clinical treatment of patients who was exposed to Iridium-192 source,promoting the repair of local severe radiation injury;through observing the chimera starus of hUC-MSCs transplantation by using STR-PCR method and detecting the expressions of VEGF with ELISA analysis,we speculated that paracrine mechanism played an important role in the repair of injury.
