Earache Analysis And Evaluation Of Intervention Effect With Hyperbaric Oxygen Therapy

Objective:To understand the earache of patients who accepted the hyperbaric oxygen therapy,analyze the impact of related factors on earache,explore the application effect of the system desensitization therapy on earache,and finally provide a theoretical basis for improving hyperbaric oxygen therapy compliance.Methods:A total of 310 patients undergoing hyperbaric oxygen therapy from two top-three hospitals in Jiujiang city were selected by convenience sampling from March to September 2017.The survey includes: general data of patients with hyperbaric oxygen therapy questionnaire,state trait anxiety scale(STAI),and visual analog scale(VAS).To analyze the related factors of earache,evaluate different degree of earache,and treat the erectile dysfunction,60 VAS patients with the first earache and scores between 4 and 6 were random allocation two groups: 1)intervention group,intervening with systemic desensitization therapy for 3 days,2)control group,accepting routine treatment procedures.3 days later,two groups of patients refilled STAI and VAS forms and further analyzed the difference between two groups in earache,anxiety,and treatment consistence.Results:1.The occurrence of earacheOf 310 patients,122 cases had earache symptom: 67 males and 55 females.The incidence rate was 39.4%.Composition ratio: the first treatment had 112 cases,accounting for 91.8%;secondary treatment had 10 cases,accounting for 8.2%.For VAS score less than 3 points,there were 60 cases(the first treatment group had 52 cases,The secondary treatment group had 8 cases)and earache happened in all pressure sections.For VAS score at 4-6 points,there were 62 cases(first treatment group had 60 cases,the secondary treatment group had 2 cases).At 0.04 MPa,therewere 60 cases,explaining 96.8%;at 0.08 MPa,there were 2 cases,accounting for3.2%.There were no cases having earache with VAS over 7 points.2.Related factors leading to earacheThere was significant difference(P <0.05)between earache patients and non-earache patients in insuflation maneuvers,age,eustachian condition,marital status,medical expense pressure,and average income per month.Logistic regression analysis of all the variables associated with earache in the general data revealed that eustachian tube ventilation and general physical condition were associated with earache.Both URI/rhinitis and general physical condition were independent factors to earache.3.Correlation of anxiety value before and after ear painThere was a linear correlation(P <0.0001)among the anxiety values obtained at normal time,undergoing hyperbaric oxygen therapy,and earache was lasting.It was suggested using anxiety value as an indirect variable to analyze the factors related to earache.4.Earache analysis of 60 cases after intervention treatmentWhen creating the groups,the scores of both groups were higher,and the difference was not statistically significant(P> 0.05).After 3 days of intervention,the earache in experimental group was significantly lower than one in the control group(P <0.05).In control group,earache varied case by case,but there was no significant difference between before and after treatment(P> 0.05).5.Anxiety analysis of 60 cases after intervention treatmentBefore 60 cases were sorted into different groups,the scores were higher in both groups,but the difference was not statistically significant(P> 0.05).The anxiety value decreased in the experimental group,while the anxiety value of the control group varied,and the difference was not significant.There was significant difference between two groups in anxiety value(P <0.05)after intervention treatment.6.Comparison of treatment adherence between two groupsAll the patients in the experimental group completed the treatment on time following doctor's advice.In control group,one patient completed the treatment with nasal spray,15 patients continued treatment after interrupted for 3 days,12 patientscontinued treatment after interrupted for one week,and 2 patients continued treatment after interrupted for 2 weeks.The compliance of the experimental group was significantly higher than that of the control group.7.Interaction between earache and anxietyThe results showed that there was an interaction between earache and anxiety after intervention treatment,when the anxiety value after the end of treatment was taken as the dependent variable.Conclusions:1.Patients of hyperbaric oxygen therapy had a higher incidence of earache during the time of high pressure.In most cases,it occurred in the first treatment.Earache varied mainly from mild to moderate,and there was no severe earache happened.2.For patients with hypertension oxygen treatment,the main factors lead to earache were upper respiratory tract infection,eustachian tube ventilation conditions,and the patient's physical condition.3.There was a positive correlation between anxiety and earache in patients accepting hyperbaric oxygen therapy.The interaction between earache and anxiety after intervention was observed.Reducing or eliminating patients' anxiety could decrease the incidence of earache and also relieve earache.4.To evaluate the eustachian tube function and physical condition before entering the cabin,select the timing of entering the cabin,ensure that the patient had the proper actions to adjust cabin pressure,monitor the computer system and the patient closely during pressurization,identify the earache and anxiety in early stage,personalized intervention,etc,were the keys to reduce the occurrence of earache in hyperbaric oxygen therapy.5.Systematic desensitization therapy reduced the patient's earache and anxiety and improving treatment compliance.
