Clinical Study On Preventing The Recurrence After Transcervical Resection Of Endometrial Polyps

Objective:Through the comparison of endometrial polyps(EP)after resection after application of drospirenone ethinyl estradiol(Yasmin)and levonorgestrel releasing intrauterine system(Man Yuele)after effect,explore the effective way of prevention and cure of hysteroscopic endometrial resection after recurrence.Methords:A prospective analysis from January 2012 to December 2014 were underwent hysteroscopic resection of endometrial polyp patients in Yancheng Maternal and Child Health-Care Center,a total of 252 patients with complete clinical data,in the premise of fully informed consent according to the wishes of patients divided into three groups,the control group were only treated with hysteroscopic endometrial resection,Yasmin group and Man Yuele group of patients.Hysteroscopic resection of endometrial polyp after application of drospirenone ethinyl estradiol(Yasmin)and levonorgestrel releasing intrauterine system(Man Yuele)adjuvant therapy for 3 months.After 6 months,12 months and 24 months,the changes of endometrial thickness,the amount of menstrual changes,and the recurrence of the patients were recorded,and the adverse reactions were observed in the observation group during the period of medication.Results: 1.Yasmin group,Mirena group and control group,the operation time was 10.34 + 3.16 min,10.42 + 3.23 min,10.53 + 2.94 min,Yasmin group,Mirena group and control group the amount of intraoperative bleeding was 8.64 + 2.89 m L,8.24 + 2.69 m L and 8.37 + 2.76 m L,three groups of patients with operation time and intraoperative blood loss were no statistical significance(P>0.05).2.Compared with the preoperative group of patients after 6 months and 12 months after surgery,endometrial thickness decreased,the difference was statistically significant(P<0.05),but in 24 months rose to the preoperative level of endometrial thickness group;Yasmin group,Mirena group of patients after 12 months after surgery,24 months endometrial thickness is lower than the control group,the difference was statistically significant(P<0.05).3.Yasmin group,Mirena group after 6 months and 12 months after surgery and postoperative menstruation 24 months abnormal rate was lower than the control group,the difference was statistically significant(P<0.05).4.The recurrence rate of 24 months Yasmin group,Mirena group 12 months after surgery,postoperative lower than the control group,the recurrence rate was lower than the Mirena group Yasmin group,the difference was statistically significant(P< 0.05).5.Yasmin group during treatment with 13 cases of adverse reactions(13.36%),Mirena group with 4 cases of adverse reactions(4.17%).The occurrence rate of adverse reactions is lower than the Mirena group Yasmin group,the difference was statistically significant(P<0.05).Conclusions: 1.Hysteroscopy in the diagnosis and treatment of endometrial polyps,in order to improve the efficacy of hysteroscopic endometrial polypectomy,postoperative adjuvant therapy is necessary.2.Hysteroscopic resection of endometrial polyps with Yasmin and Mirena treatment,the endometrial thickness,reduce menstruation,reduce postoperative recurrence.3.Hysteroscopic resection of endometrial polyps after Mirena adjuvant therapy compared with Yasmin,can effectively reduce the safety operation within two years after the recurrence rate and higher.
