Objective:Cervical cancer is one of the most common gynecological cancer,strengthen the prevention and treatment of cervical cancer also imminent.The purpose of this study was to compare the TCT and HPV DNA test in cervical cancer screening between the SiHong,JiangSu province and ShangHai post and telecommunications system.Methods:From July 2015 to July 2016,1719 women in Sihong County,Jiangsu Province and1259 cases in Shanghai Post and Telecommunication System was included according to the inclusion and exclusion criteria.In this study,TCTand HPV-DNA were used to screen for the follow-up examination and treatment of HPV-DNA positive,TCT positive and macroscopic examination of suspicious cases.SPSS statistical software was used to analyze the relationship between various factors and the occurrence of cervical cancer,to explore the risk factors of cervical cancer.Results:1.The mean age of the two groups of Sihong County and Shanghai Post and Telecommunication System were 47.95 ± 11.83 and 46.67 ± 11.38 years-old respectively.Compared with the Shanghai post and telecommunications system women,Jiangsu Sihong women were older with more heavy body mass index,more in the postmenopausal state,less in working condition,lower income,earlier first pregnancy,more Children and lower prevalence of hypertension,diabetes(P <0.001).2.The incidence of cervicitis in Sihong County and Shanghai Post and Telecommunication System was 16.46% and 7.86%(P <0.01)respectively.The infection rates of HPV were 10.99% and 5.00%(P <0.01)respectively.The positive rates ofCIN?and cervical cancer were 0.93% and 0.24 %(P < 0.05)respectively,and the percentage of HPV + TCT-was 8.49% and 2.54%(P <0.01)respectively.All indexes in Sihong County,Jiangsu Province were higher than that in the Shanghai Post and Telecommunications System.3.The univariate analysis of cervical lesions in Sihong area of Jiangsu showed thatthe following 8 riskfactors were significantly correlated with the cervical lesions,including the first sexual intercourse age less than 20(P=0.012),the first pregnancy age less than 20 years-old(P=0.017),low level of culture(P=0.033),low income groups(P=0.021),maternal time more than 2 times(P=0.024),poor physical health condition for sexual partner(P=0.044),HPV infection(P< 0.001)and uterine cervicitis(P=0.001).Multivariate regression analysis showed that HPV infection and maternal time more than 2 timeswere independent risk factors for cervical lesions(HPV positive: OR 18.570,95% CI 9.762-35.325,P <0.001;maternal time more than2 times: OR 2.036,95% CI 1.033-4.013,P = 0.04).The univariate analysis of cervical lesions in ShangHai post and telecommunications system showed thatthe following 4risk factors were significantly correlated with the cervical lesions,including the first sexual intercourse age less than 20(P=0.006),low income groups(P=0.015),HPV infection(P<0.001)and uterine cervicitis(P=0.004).Multivariate regression analysis showed that HPV infection and uterine cervicitiswere independent risk factors for cervical lesions(HPV positive: OR 287.122,95% CI 93.996-877.054,P < 0.001;uterine cervicitis: OR 0.142,95% CI 0.044-0.459,P= 0.001).Conclusions:For Si Hong JiangSu province,there were significant higher rate at the items of cervicitis,HPV infection and high-level CIN disease rate comparing to ShangHai post and telecommunications system,and there was no difference at the item of cervical cancer incidence.The first sexual intercourse age less than 20,low income groups,HPV infection and uterine cervicitis were common risk factors leading to cervical lesions both in Sihong area of Jiangsu and Shanghai Post and Telecommunications System.Among them,HPV infection and maternal time more than 2 timeswere independent risk factors for cervical lesions in Sihong area,and HPV infection and uterine cervicitiswere independent risk factors for cervical lesions in ShangHai post and telecommunications system.In addition,low level of culture,maternal time more than 2 times,the first pregnancy age less than 20 years-old,poor physical health condition for sexual partner and uterine cervicitiswere significantly correlated with the incidence of cervical lesions in Sihong area in Jiangsu.
