Purpose:To investigate and analyze on the surgical treatment cost of chronic dacryocystitis in one hospital of Changchun;get the analysis data of the cost of different treatment for patients with chronic dacryocystitis,by means of the theories and methods of health economics,compare the differences of medical treatment costs in different surgical methods and compare differences of costs in different medical insurances in the cases with the same quality of treatment;understand the treatment effect with different treatment methods.Let patient pay attention to the effect of treatment while considering the economic burden of the disease.To provide theoretical basis for the selection of different surgical treatment options for chronic dacryocystitis,and to provide data support for the medical insurance management department's cost control.Method:The patients with chronic dacryocystitis and nasolacrimal duct obstruction treated in the Department of Ophthalmology of a hospital between April 2015 and June 2016 were selected as the subjects of investigation and grouped according to different surgical procedures.1.Literature research.The related literatures were reviewed to understand the current domestic and international experiences of treating chronic dacryocystitis.2.Questionnaire survey.Organize the expert to design the research plan,determine the content of the questionnaire,make and design the questionnaire.Collect and calculate patient information with Excel software.The direct medical cost,the indirect medical cost and the total cost of the patients were observed,and the cost-effectiveness analysis was carried out.Using the method of disease cost research,the formula is:disease cost=direct medical cost+indirect cost.3.Cost survey.Analysis of different treatment costs,SPSS18.0 software was used for data processing,the mean and standard deviation with measurement data,count data usingc2 test,there is a significant difference when P<0.05.Result:1.Hospitalization costs and days of hospitalization for patients with nasal dacryocystorhinostomy and catheterization were significantly higher than those of other surgical procedures and days of hospitalization.?P<0.01??2.There is a significant difference in the difference of self payment among different types of medical insurance?P<0.01?,The cost of self payment of patients with Rural medical insurance and patients without medical insurance were significantly higher than that of patients with urban medical insurance.3.There was significant difference in the difference of the examination fee among different types of operation?P<0.05?,and there was no significant difference in other items?P>0.05?.4.Among the patient with good satisfication,the cost of lacrimal canalicular incision and catheterization was 8968 yuan,which was less than the cost of recanalization and catheterization of lacrimal duct?13303 yuan?and less than the cost of nasal endoscopic dacryocystorhinostomy plus catheterization?16046 yuan?.Conclusion:1.The cost and length of hospitalization of endoscopic anastomoses were significantly higher than those of the other two methods.2.There is no significant difference among the different types of Medicare patients in the total hospitalization expenses,but for the patient's own expense,the costs of patients with Rural medical insurance and patients without medical insurance are higher than that of patients with urban medical insurance.3.The examination cost of lacrimal duct incision plus catheterization was significantly higher than that of nasal endoscopic dacryocystorhinostomy plus catheterization.4.The traditional lacrimal canalicular incision and catheterization is superior to minimally invasive endoscopic dacryocystorhinostomy in the cost effectiveness comparison.But nasal endoscopic dacryocystorhinostomy has the advantages of less damage to the body and faster recovery after operation.The doctor should consider the patient's condition,the economic condition,the individual will and so on to choose the surgery method.
