Purpose:In this study,by comparing the traditional supine delivery and application of delivery ball assisted delivery of primipara in uterine contraction pain and anxiety,the second stage of labor time,perineal laceration,2 hours of postpartum hemorrhage and neonatal 1 min and 5 min Apgar score to explore the clinical value of delivery ball in primipara natural childbirth,aiming at improving the normal delivery rate,reducing the unnecessary cesarean section rate,reducing the physical and psychological injuries caused by delivery,and changing the shadow and fear caused by traditional childbirth.Methods:A total of 240 primiparas with single head position who received natural childbirth in our hospital from July 2016 to February-2017 were selected and divided into the experimental group(120 cases)and the control group(120 cases)according to the principle of knowing the truth and voluntarily.The test group used assisted delivery in maternal childbirth ball during labor;the control group using conventional supine or semi supine delivery.Epi Data 3.2 was used for double data entry and collation,and SPSS 22 was used for the collation and data analysis.The continuous variables were described by the median(interquartile range),count data using rate,constituent ratio(%).Rank sum test or chi square test was used to compare the differences between groups,with p<0.05 as a the statistical significance of the critical value.Through the above statistical methods to analyze the differences between two groups of maternal uterine contraction pain and anxiety,the second stage of labor time,perineal laceration,1 minutes and 5 minutes between the neonatal Apgar score and birth outcomes of 2h after childbirth postpartum hemorrhage.Results:1.There were differences in pregnancy(P =0.026),education,occupation and income between the two groups.(P < 0.001).Moreover,the weight of the newborn is different between groups(P =0.020).2.The degree of pain and anxiety of uterine contraction in the control group was higher than that of the test group(P< 0.001).In the test group,the degree of pain and anxiety of the uterine contraction after the delivery of the ball was lower than before(P< 0.001).3.The comparison between the experimental group and the control group about the related indicators of maternal delivery showed that the difference between the 2 hours postpartum hemorrhage volume(ml)(P<0.001)and the perineum tear degree was statistically significant(P < 0.001).4.The influencing factors of postpartum 2 hours bleeding volume were analyzed by multivariate logistic regression analysis.The results showed that the degree of perineal tear,age and the use of delivery balls were the influencing factors of 2 hours postpartum hemorrhage.In the degree of perineal laceration,the risk of abnormal postpartum hemorrhage of perineum lacerated parturients is 20.329 times as high as that of the full perineum(95%CI:3.962~104.322).The risk of abnormal postpartum hemorrhage in parturients was 28.451 times as high as that of the full perineum parturient(95%CI:5.377~150.543).Then,the risk of abnormal postpartum hemorrhage by 1 years of age increased by 1.242 times(95%CI:1.067~1.446).At last,the risk of abnormal postpartum haemorrhage was 50.482 times as high as that of a parturient who used a birthing ball without a delivery ball(95%CI:13.655~186.633).5.Disordered multi classification logistic regression analysiswas used to explore the influence factors of the degree of perineum tear in parturients,and the results showed that whether the use of delivery ball are the factors influencing maternal perineal tear degree,and the possibility of using the maternal childbirth ball receing lateral incision for 0.002 times without the use of maternal childbirth ball(95%CI:0.000~0.057).6.The Apgar score of the newborn in the test group(1 minutes,5 minutes)was higher than that of the control group,and the difference was statistically significant.(P < 0.001,P = 0.044).7.Analysis of the puerpera's cognition of the birthing ball showed: the difference in the distribution of understanding between the two groups was statistically significant(?2=61.478,P<0.001).The main reason why the control group was not willing to use the ball was not to believe the effect of the ball,accounting for 51.70%.There are 45 people who don't want to try someting new,which is 37.5percent.Conclusion:Through the use of primipara childbirth ball assisted labor can effectively reduce uterine pain and anxiety fear.In addition,the proper use of delivery ball also can reduce maternal perineal laceration degree,reduce the risk of postpartum hemorrhage 2 hours increased.At the same time also can reduce the incidence of neonatal asphyxia,and improve maternal birth outcomes,has important and far-reaching the significance of the protection of the health of mothers and babies.
