Analysis Of 346 Cases With Acute Basal Ganglia Infarct

Purpose: Acute cerebral infarction(ACI)is a kind of common cerebrovascular disease with high mortality and disability rate which seriously affects patient's health.Basal ganglia region is the most common site of ACI.Because Basal ganglia region includes lots of fibre bundle such as corticospinal tract,ACI at this region easily cause dysfunction of limbs,but it can not affect consciousness and speak.This study is aimed at investigating epidemiological and clinical features of acute basal ganglia region cerebral infarction.Since adjuvant therapy of ACI includes percutaneous electrical nerve stimulation,acupuncture,and medicine,this study also compared prognosis of ACI among these three therapies,and investigated influence factors of prognosis.Methods: Data of 346 patients with acute basal ganglia infract who are hospitalized in Neurology Department of Red cross hospital from Jan.2015 to Jan.2017 were collected.The data collected included general information(sex,age,admission time,smoking history,and drinking history),admission information(disease time,body measurement index,blood biochemical index,and whether complicated with other diseases),and treatment information(methods of treatment,Barthel index before and after treatment,Fugl-Meyer index before and after treatment,CSS index before and after treatment,and outcome of treatment).Then the data were analyzed using corresponding statistical methods.Results:I.General information:(1)Age and sex distribution: The average age of these patients was 69.37±7.50 years old,with 46.5% in the age range of 70-79 years old.Proportion of man was higher than that of woman with a male: female ratio of 1.99:1.(2)Admission time: 38.2% patients went into hospital in first quarter(January to March),and 34.1% patients went into hospital in fourth quarter(October to December).II.Admission information:(1)Disease time: 67.1% patients were treated within 24 hours after ACI outbreaking.(2)Body measurement: In this study,overweight patients were the most accounting for 57.8%,and obese patients were the least,with a proportion of 2.9%.The height and weight of men were higher than those of women(P<0.001),but there was no difference of BMI between men and women(P=0.743).(3)Biochemical index: The abnormal rate(94.5%)of LDL-C was the highest among TG,TC,HDL-C,LDL-C,and HCY,however,the rate(2.6%)of HDL-C was lowest.There was no difference between men and women at the level of blood lipid,TG,TC,HDL-C,LDL-C,and HCY(P>0.05).(4)Whether complicated with other diseases: In this study,310 patients(89.7%)had high blood pressure,143 patients(41.3%)had diabetes,and 152 patients(43.1%)had coronary heart disease.(5)Smoking and drinking: 147 patients(42.5%)had smoking history,and 76 patients(22%)had drinking history.III.Treatment information:(1)Treatment method: 108 patients(31.2%)chose percutaneous electrical nerve stimulation as adjuvant therapy method,116 patients(33.5%)chose acupuncture,and 122 patients(35.3%)only had medicine as treatment method.(2)Change of blood biochemical index after treatment: 66.1% patients with abnormal TG levels,97.7% patients with abnormal TC levels,96.0% patients with abnormal LDL-C levels,and 34.6% patients with abnormal HCY levels recovered to the normal levels after treatment.(3)Functional evaluation: After treatment,the score of Barthel and Fugl-Meyer increased,but the score of CSS decreased(P<0.001).(4)Prognosis: Treatment of 114 patients(32.9%)were significantly effective,treatment of 110 patients(31.8%)were effective,and treatment of 122 patients were inefficacy.IV.Functional scoring comparison: Before treatment,there were no statistical difference among these three groups(patients treated with percutaneous electrical nerve stimulation,patients treated with acupuncture,and patients treated with medicine)on the index of Barthel,Fugl-Meyer,and CSS(P=0.471,P=0.116,P=0.308).However,after treatment,the score of Barthel and Fugl-Meyer increased,and the score of CSS decreased in each group(P<0.001).There were statistical difference among these three groups at the prognosis of patients(p=0.001).Among them,there was no statistical difference between patients treated with percutaneous electrical nerve stimulation and patients treated with acupuncture(P>0.05).However,both these two groups were different with patients treated with medicine(P<0.05).V.Risk factors of treatment effect: Unconditional multiple logistic regression analysis showed that drinking,disease time,and percutaneous electrical nerve stimulation were the risk factors of prognosis of ACI.Conclusions:(1)Most of the patients with acute basal ganglia infract were of the age range of 70 to 79 years.The number of men were large than that of women.(2)First quarter and fourth quarter had largest number of patients.(3)Most of the patients with acute basal ganglia infract had high blood pressure.(4)The prognosis of patients whose disease time were less than 12 hours were better than that of patients whose disease time were 12~36 hours.(5)The prognosis of patients treated with percutaneous electrical nerve stimulation were better than that of patients treated with medicine.
