Objective:1.To study the CT features of brain structure in patients with paranoid schizophrenia and their association with age,duration and clinical symptom score.2.To discuss the effects of smoking on brain structure in patients with paranoid schizophrenia.Methods:1.Select 90 cases of schizophrenia paranoid patients,from October2014 to January 2016,diagnosed in Tianjin Anding Hospital as the research objects.Retrospective analysis of the CT images and matched control group in age,gender,education level and other clinical characteristics,whose patients had a headache or dizziness to Tianjin Hospital for treatment.Measure the brain structure of two groups of subjects by linear method.The measuring lines included indicators related to the frontal,temporal lobe atrophy,and indicators related to ventricular dilation.Forehead,temporal lobe atrophy of the relevant indicators include:Double double frontal longitudinal spacing(DFS),left and right lateral fissure width(LLFW,RLFW),temporal lobe hook spacing(THS).The relevant indicators of ventricular enlargement include : the width of the third ventricle(TVW),Harrington value(HI),Ventricular index(VI),Lateral ventricle index(LVBI),Frontal horns index(FHI).Analyze all data by SPSS20.0 statistical software.Compared the difference of brain structure of the two groups,using independent sample t test;To study the relationship between the abnormal brain structures and age,course of disease and PANSS score in the study group,respectively by Pearson correlation analysis.2.According to smoking,the study group of 90 patients with paranoid schizophrenia were divided into smoking group and non smoking group.Compared the difference of brain structure of the two groups,using independent sample t test.Results:(1)Comparison of the brain structure of two groups : from the brain structure of the study group,we can get,the double frontal longitudinal spacing(DFS),left and right lateral fissure width(LLFW,RLFW),double the amount of longitudinal spacing(DFS),temporal lobe hook spacing(THS),the width of the three ventricle(TVW),Harrington value(HI)were significantly higher than that from of the normal controlgroup.Comparison of independent sample t test,the differences were statistically significant(P<0.05);in the study group,ventricular index(VI)and frontal horns index(FHI)were lower than the normal control group,Comparison of independent sample t test,the differences were statistically significant(all P<0.05),but there was no significant difference between the two groups(P>0.05)in the cerebral ventricle index(LVBI).(2)Correlation analysis of the brain structure and PANSS score of the study group:Left or right lateral fissure width(LLFW,RLFW)and temporal lobe hook spacing(THS)were positively associated with positive symptoms scores(P<0.05).Left or right lateral fissure width(LLFW,RLFW),temporal lobe hook spacing(THS),the width of the three ventricle(TVW),Harrington value(HI)were positively correlated to the negative symptoms scores(P<0.05);The negative symptoms score were negatively correlated to ventricular index(VI)and frontal horns index(FHI)(P<0.05).Others such as pathological score or the total score of PANSS,were no significant correlation with the measured values of brain structure(P>0.05).(3)The relationship between brain structure changes with age in study group: in the study group,left and right lateral fissure width(LLFW,RLFW),the double frontal longitudinal spacing(DFS),temporal lobe hook spacing(THS),the width of the three ventricle(TVW),Harrington value(HI)were positively correlated with age(P<0.001).The other indexes,like ventricular index(VI),ventricular body index(LVBI)and frontal horns index(FHI)were negatively correlated with age(P<0.05).(4)The relationship between brain structure changes and course of disease in study group: the study group's left and right lateral fissure width(LLFW,RLFW),double the amount of longitudinal spacing(DFS)were positively correlated with the course of disease(P<0.01).The other indexes have no correlation with the course of the disease(P>0.05).(5)The relationship between brain structure changes and smoking in the study group: left and right lateral fissure width(LLFW,RLFW),double the amount of longitudinal spacing(DFS),the width of the three ventricle(TVW)were significantly higher than non smoking group,and ventricular index(VI),frontal horns index(FHI)were lower than non smoking group,the differences were statistically significant(P<0.05),but temporal lobe hook spacing(THS),Harrington value(HI),ventricular body index(LVBI)was no significant difference between the two groups(P>0.05).Conclusion:(1)Compared with normal people,the brain structure in paranoid schizophrenia patients is obviously abnormal.(2)In Paranoid schizophrenia patients,the positive symptoms were positively correlated with the degree of temporal lobe atrophy.The more obvious the temporal lobe atrophy,the more marked the positive symptoms.(3)In Paranoid schizophrenia patients,the degree of ventricular enlargement and atrophy of frontal or temporal lobe is correlated with age,suggesting that with the increase of age,brain atrophy in patients of paranoid schizophrenic gradually increased.(4)The degree of brain atrophy in frontal and temporal lobes of paranoid schizophrenia patients aggravated gradually with the persistent of the course of the disease.This may be the key factor of the longer duration with paranoid schizophrenia patients.(5)Smoking has a certain correlation with the changes of brain structure in paranoid schizophrenia patients.
