ObjectiveTo analyze the clinical application value of combined detection of AO-F(acridine orange fluorescene),BTA(bladder tumor antigen)and FISH(fluorescence in situ hybridization)for diagnosis of bladder cancer.And to provide theoretical basis for non invasive method in evaluating the bladder tumor.Methods1.To study the clinical data of 109 urinary tract diseases patients coming to the Second Hospital of Tianjin Medical University from March 2016 to November2016.82 patients were diagnosed with bladder urothelial carcinoma according to the final pathology report.27 patients were benign urinary tract diseases as controls.2.Collect and group together the urine specimens,and AO-F,BTA and FISH were used to detect these specimens.3.Analyze the clinical application value of AO-F,FISH and BTA in bladder carcinoma,and investigate the diagnostic specificity and sensitivity of single detection.4.We analyzed the sensitivity,specificity and the positive predictive value of all the combined examining methods.Receiver operating characteristics(ROC)curve was drawn by SPSS18.0 software.Investigate three methods diagnostic value in the detection of bladder carcinoma by the area under the receiver operating characteristic(AUROC).Results1.Compared with the 3 methods,overall sensitivity of AO-F,BTA and FISH analyses reached 62.2%,69.5% and 74.4%,respectively,and overall specificity was92.6%,77.8% and 89.9%.Using area under the ROC curve as performance measurement,the results show the FISH has the highest diagnostic value in these 3methods.2.In low-grade and staging BCa,the sensitivity of AO-F was lower than high-grade and staging,the difference was statistically significant.3.By analyzing the results of combination of three method detecting bladdercancer,the combined detection was divided into series and parallel.In combined detection,if all examination results are positive and the overall results are positive,it will be series;in the combined detection,if there is at least one detection result is positive and the overall results are positive,it will be parallel.The specificity of all series and parallel combinations is 100%.Among all series examination,the series of BTA and FISH examination is the combination with the highest sensitivity,which is73.2%.Based on AUROC,it can be concluded that AO-F paralleling FISH examination has the biggest diagnosis value for bladder cancer.Conclusion1.In this experiment,the urine sample is easy to be collected,with a high feasibility.2.AO-F,BTA,FISH are of clinical significance for the diagnosis of bladder cancer.3.FISH has the highest diagnosis value in detecting bladder cancer.4.The sensitivity of AO-F has a positive correlation with clinical staging and pathologic grading.5.Among various combinations of these three detection methods,AO-F paralleling FISH examination has the biggest diagnosis for bladder cancer.So it is suggested that patients should receive AO-F and FISH examination and reduce the using frequency of cystoscopy.
