Objective:To analyze the clinical features and prognostic factors of the newly diagnosed Hodgkin's lymphoma(HL)patients.Method:From January 2007 through December 2016,we collected the medical records of 117 newly diagnosed HL patients from our hospital who were pathologically confirmed,including the patients' clinical manifestation,laboratory examination,treatment,the short-term efficacy and long-term survival.Furthermore,Kaplan-Meier method,the Log-rank test and Cox proportional hazard model were applied to analyze the prognosis.All datas were analyzed retrospectively with SPSS software version 19.0.Results : The study comprised 64 male and 53 female patients,and the ratio of male:female was approximately 1.21:1.The median age of the 117 patients was 28years(range: 6-82 years),and the median survival period was 37 months(range:1-118months).The 5-year overall and event-free survival rates were 86.7% and 56.7%.HL occurred most frequently in the neck superficial lymph nodes(72patients,61.5%).Classical Hodgkin's lymphoma(CHL;101 patients,86.3%)was the most common pathologic subtype,including 57 patients with nodular sclerosis Hodgkin's lymphoma(NSHL),21 patients with mixedcellularity Hodgkin's lymphoma(MCHL),19 patients with lymphocyte rich Hodgkin's lymphoma(LRHL)and 4 patients with lymphocyte depleted Hodgkin's lymphoma(LDHL).There were followed by 6 patients with nodular lymphocyte predominance Hodgkin's lymphoma(NLPHL)and 10 patients with undifferentiated subtype.Clinical stage ? lymphoma was observed in 13 patients,stage ? in 46,stage ? in 30,and stage ? in 28.Univariate analyses showed that age,Ann Arbor stages,B systems,extranodal involvement,albumin level and the short-term efficacy which were prognositic factors for 5-year OS,and the last two factors were prognositic factors for 5-year EFS.Multivariate analyses showed that age?45 years,B systems,extranodal involvement and failure to achieve complete remission(CR)or uncertain complete remission(CRu)were significant risk factors for overall survival(P<0.05).Therefore,according to the above-mentioned adverse prognostic factors,117 patients were regrouped as low,intermediate and high risk.The 5-year OS rate of three groups were 98.5%,72.6%and 26.7%.And the OS between three groups was significantly different(P<0.05).Conclusion:HL was found to have a high incidence in young male patients but the curative efficacy is optimal and the survival period is long.Age ? 45 years,B systems,extranodal involvement and CR or CRu failure after first-line treatment were significant poor prognostic factors for OS according to the multivariate analysis.According to the above-mentioned factors,newly diagnosed patients with HL can be divided into different risk stratifications,including low risk,intermediate risk and high risk,which can apply to predict the prognosis of HL patients to a certain extent.
