Biological Behavior Reseach Of Sentinel Lymph Node Micrometastases In Breast Cancer

Objective: To investigate the biological behavior and prognostic significance of sentinel lymph node micrometastasis in breast cancer.Methods: A retrospective study was conducted on the clinical data of 3211 patients with breast cancer from September 2013 to August 2016 in Fujian Union Medical College Hospital,where 442 cases of sentinel lymph node biopsy were performed by routine pathology combined with continuous biopsy Sentinel lymph nodes,excluding sentinel lymph node macrometastasis and isolated tumor cells in patients.Sentinel lymph node micrometastasis group has 44 cases,sentinel lymph node negative group has 314 cases.To compare sentinel lymph node micrometastasis and sentinel lymph node negative group age,menstrual status,histological grade,primary tumor size,ER,PR,Ki67,Her2,disease free survival rates.Kaplan-Meier method was used to plot the survival curve,and the survival difference was compared by log-rank test.Result:1.The sentinel lymph node biopsy accounted for 13.8%(442/3211)in the same period,and the ratio of sentinel lymph node micrometastasis to sentinel lymph node biopsy was 10.0%(44/442),sentinel lymph node negative was 71%(314/442).2.In the histological grade,the poorly differentiatedin of breast cancer was 88.6%(39/44),which was higher than that 68.2%(214/314)in breast cancer lymph node negative group.There was significant difference between the two groups(P = 0.005).In the comparison of high expression of Ki67(> 50%),the high expression of Ki67 in breast cancer micrometastasis group was 86.4%(38/44),which was higher than that 33.8%(106/314)in breast cancer lymph node negative group.There was significant difference between the two groups(P = 0.001).There was no significant difference between the two groups in age,menstrual status,ER,PR,Her2 and tumor size.P> 0.05.3.The median follow-up time was 36 months,and the sentinel lymph node micrometastasis group had 2 ralapse cases,sentinel lymph node negative group has 16 cases..There was no significant difference in DFS between the two groups(P> 0.05).Conclusion:1.The detection rate of sentinel lymph node micrometastasis in sentinel lymph node was 10.0%(44/442)by routine pathology combined with continuous sectioning.2.The proportion of poorly differentiatedin and Ki67 high expression in sentinel lymph node micrometastases was significantly higher than that in sentinel lymph node negative group,there was significant difference between the two groups(P <0.05).It may suggest that the sentinel lymph node micrometastasis group has worse biological behavior.3.The median follow-up time was 36 months,and there was no significant difference in DFS between sentinel lymph node micrometastasis group and sentinel lymph node negative group(P<0.05).
