Objective:To explore the risk factors of early recurrence after radical resection of early hepatocellular carcinoma,and to provide reference for the clinical treatment of early hepatocellular carcinoma.Methods: A retrospective analysis of 114 patients with primary hepatocellular carcinoma who underwent radical resection for the first time from January 1,2011 to December 31,2014,with complete follow-up data.Using outpatient and telephone follow-up combination of the way to understand whether patients with recurrence and recurrence time and other information,follow-up deadline for January 1,2017.According to the recurrence time of patients were divided into two groups,early recurrence group(1 year of liver cancer recurrence)and non-early recurrence group(1 year,no recurrence of liver cancer).Diabetes mellitus,diabetes mellitus,history of alcohol consumption,HBs Ag,HBV-DNA,GGT,ALT,AST,ALB,and so on.The factors influencing the early recurrence of small hepatocellular carcinoma after radical resection were: gender,age,PT,Fib,AFP,CEA,Ca19-9,Child-Pugh classification,BCLC stage,portal hypertension,tumor histological differentiation,tumor size,cirrhosis,vascular invasion,hepatic invasion,Liver resection,hilar occlusion,operation time,intraoperative blood loss,intraoperative blood transfusion,postoperative preventive TACE,antiviral therapy and other factors were analyzed.SPSS18.0 statistical software for data analysis,the patient's tumor-free survival rate using Kaplan-Meier method,and draw the survival curve.The multivariate analysis was performed using the logistic regression model for multivariate analysis of the P value <0.05 using the ?2 test or Fisher exact probability analysis.Results:In the group of 114 patients with radical hepatectomy after radical resection,the total number of recurrence during follow-up was 52,the overall recurrence rate of 45.6%;1 year of recurrence of 15 people,the recurrence rate of 13.2%.Univariate analysis showed that HBV-DNA(P = 0.015),AFP(P = 0.018)and tumor size(P = 0.008) were the risk factors for early postoperative recurrence in patients with small hepatocellular carcinoma.(P <0.05),but not with gender,HBs Ag,CEA,Ca19-9,BCLC staging,portal hypertension,cirrhosis,vascular thrombosis,hepatic invasion,hepatic perineal invasion,surgical modality,postoperative preventive TACE,antiviral therapy <0.05).Multivariate analysis showed that tumor size(P = 0.042)and HBV-DNA(P = 0.028)were independent risk factors for early postoperative recurrence in patients with small hepatocellular carcinoma.AFP was not an independent risk factor for early postoperative recurrence of patients with small hepatocellular carcinoma(P> 0.05).Conclusions:1.HBV-DNA,AFP,tumor size is a risk factor for early postoperative recurrence of patients with early hepatocellular carcinoma;2.HBV-DNA,tumor size is an independent risk factor for early postoperative recurrence of patients with early hepatocellular carcinoma.
