Objective To explore the clinical effect of minimally invasive pedicle screw fixation(hollow pedicle screws with lateral holes)combined with bone cement for the treatment of thoracolumbar burst fracture in elderly patients with osteoporosis.Methods A total of 80 patients with osteoporotic thoracolumbar burst fractures who had no nerve damage were enrolled in our hospital from June 2013 to June 2015.They were randomly divided into experimental group and control group.Each group was randomly divided into experimental group and control group.Forty patients in the experimental group were treated with minimally invasive pedicle screw fixation combined with vertebrae;Forty patients in the control group were treated with minimally invasive pedicle screw fixation.The two groups were compared in terms of perioperative indexes(operation time,intraoperative blood loss,hospitalization time),pain relief degree [visual analog scale(VAS)],cobb angle of the fixed segment,loss of vertebral body heightt.All the dates were made statistical analysis by SPSS19.0.Results All the patients were followed up,average of 16 months.There was no significant difference between the two groups in age,sex ratio,injury segment,bone mineral density,P> 0.05.The experimental group was higher than the control group(P < 0.05)during the operation,but the difference in the amount of blood and the length of hospitalization was not statistically significant(P > 0.05)..The VAS,cobb angle of the fixed segment and loss of vertebral body height at 7-day,1-,6-,12 month were significantly improved in all the patients(P<0.05).The test group had significantly better improvements in VAS,cobb angle of the fixed segment and loss of vertebral body height at 7-day,1-,6-,12 month than the Control group(P<0.05).None of the patients had complications of neural injury,Bone cement leakage or poor wound healing.Conclusion The minimally invasive pedicle screw fixation combined with bone cement treatment of elderly patients with osteoporosis thoracolumbar burst fracture,with short operation time,the advantages of small injury,quicker and better recovery,and correction and maintenance of vertebral reduction effect is good Ideal for osteoporotic thoracolumbar burst fracture surgical treatment.
