Objectives:1.Action research method was adopted to revise and improve mindfulness-based intervention model for women in pregnancy termination for fetal abnormality to improve its clinical applicability and compliance of research objects.2.To evaluate the effect of mindfulness-based intervention to raise the level of mindfulness and alleviate anxiety,depression,and labor pain of women in pregnancy termination for fetal abnormality.Methods:1.A mindfulness-based intervention model for women in pregnancy termination for fetal abnormality has been formulated on the basic of former study,then 20 maternal patients were selected to test feasibility of the model according to action research model,which involved 4 cycles of action planning,taking action,observing and reflecting.Data were collected and analyzed in combination of qualitative research and quantitative research.2.Quasi-experimental study was employed.One hundred and one cases of maternal patients were recruited,and then randomly divided into two groups according to the number of wards.A total of 89 cases completed the study,41 cases in test group and 48 cases in control group.Patients in both groups received conventional care,but the test group added the mindfulness-based intervention which was delivered from admission to discharge after 3 weeks.Data were collected at baseline and immediately after intervention with MAAS,SAS and SDS to evaluate the effect of mindfulness level and emotional state.VAS was used to evaluate the labor pain remission after induction of labor.Results:1.4 cycles of the action research model was adopted to improve and perfect the mindfulness-based intervention model for women in pregnancy termination for fetal abnormality.The revised model mainly includes intensive short mindfulness training in hospital and self-practice mindfulness after discharge.The maternal patients' compliance on mindfulness practice increased after model revised and the results presented by observation and interview also objectively evaluate the effectiveness and feasibility of the mindfulness-based intervention model.2.The results of experiment study indicated that:(1)There's a significant difference(t=2.322,P<0.05)between experiment group and control group in MAAS at post-test.The experiment group showed a significant difference in mindfulness level(t=-6.261,P<0.001)after mindfulness training,and no significant difference(t=-1.683,P>0.05)was found in the control group.(2)There's no significant difference(t=-1.310,P>0.05)between experiment group and control group in SAS at post-test.Both experiment group and control group showed a significant difference(t=11.997,P<0.001;Z=-3.425,P<0.05)in anxiety symptoms after intervention,whereas the change scores between experiment group and control group showed a significant difference(t=2.295,P<0.05)at post-test.(3)Both experiment group and control group showed a significant difference(t=6.883,P<0.001;t=2.370,P<0.05)in depressive symptoms after intervention,whereas there's a significant difference(t=-2.082,P<0.05)between experiment group and control group in SDS at post-test.(4)There's no significant difference(Z=-0.659,P>0.05)between experiment group and control group in VAS at post-test.Conclusions:1.The model of mindfulness-based intervention which was perfected by action research method can better adapt to clinical objective conditions,attaches great importance to the maternal patients' feelings and integrate the actual requirement of them,therefore it is more suitable for the actual work.After improving the plan,the compliance of research objects and the effectiveness of the proposed model were enhanced.2.The mindfulness-based intervention can improve mindfulness level of women in pregnancy termination for fetal abnormality and alleviate their anxiety and depression symptom.But the effect of labor pain do not reach statistical significant level and need further research.
