Objective:Based on the MRI of pelvic floor and finite element analysis,the geometry and relative stress of the pelvic floor muscle in the levator ani muscle were analyzed during storage and leakage state,to explore research methods for stress distribution law of levator ani muscle in women with moderate to severe stress urinary incontinence(SUI),in order to further explain the mechanism of the occurrence and development of SUI disease,and to provide a new biomechanical theoretical basis for the technical improvements of following pelvic floor repair treatment.Methods:Selected 1 cases of female patients with moderate to severe SUI in our hospital,using 3.0T magnetic resonance scanner(GE Signal 3.0T magnetic resonance machine)to scan the pelvic tissue,take supine position,for transverse plane magnetic resonance scanning,since the bladder central to the vaginal opening,a thick layer of 2.0mm,Collect the pelvic MRI images,DICOM format images,including pelvis,levator ani muscle,obturator muscle,lead MRI data into Mimics10.0 application software for three-dimensional reconstruction,3D geometric model is established including levator ani,obturator muscle and pelvis.And with the corresponding urodynamic results(such as abdominal pressure,abdominal pressure leak point pressure etc.)as a necessary constraint,lead the three-dimensional geometric model into finite element analysis software Ansys11.0 based on the method of reverse engineering,The finite element model of levator ani muscle can be used to reflect the biomechanical characteristics of pelvic floor tissues in women with moderate to severe SUI,and to calculate the stress distribution of pelvic floor muscles under the condition of moderate to severe SUI during storage and leakage state.Results:1.A finite element model of levator ani muscle was established based on reverse engineering method,which can reflect the stress distribution of pelvic floor tissue in women with moderate to severe SUI.2.The displacement and stress distribution nephogram of pelvic floor muscles during storage and leakage state in women with moderate to severe SUI were obtained.3.The changes of stress and displacement distribution of pelvic floor muscles under different abdominal pressure and elastic modulus were compared and analyzed.Conclusion:Based on pelvic floor MRI image data and combined with the corresponding results of the measurement of urinary dynamics,discussion finite element stress analysis methods for pelvic floor muscles in women with moderate to severe SUI,it can reflect the distribution law of stress distribution of pelvic floor in this pathological state,and effectively simulate the stress and displacement changes of pelvic floor muscle in moderate to severe SUI storage and urine leakage,it can provide some theoretical reference for the research of SUI disease mechanism and the improvement of treatment technology.
