Objective: To compare the safety and efficacy between flexible ureteroscope lithotripsy(FURL)and percutaneous nephrolithotomy(PCNL)in treating proximal ureteral calculi(diameter≥10mm).Method: The clinical data of 118 patients who underwent FURL and PCNL for impacted proximal ureteral calculi(diameter≥10mm)from January 2013 to October 2015 were retrospectively analyzed.52 patients were treated with FURL and 66 patients were treated with PCNL.The operation time,stone-free rate,perioperative hemoglobin decrease,perioperative WBC,surgical complications rate and length of stay were compared retrospectively between two groups.Results: All the operations were finished successfully.There was no statistical significant difference in stone-free rate and surgical complications rate between the two groups(P>0.05),but 2 patients appeared serious complications,including 1 case of patients with septic shock,another 1 patients kidney continuous bleeding in PCNL group.The operation time of FURL group was longer than PCNL group,but the visual analogue scale(VAS)score on first postoperative day,perioperative hemoglobin decrease,perioperative white blood cell count(WBC)and length of stay after operation of FURL group were evidently lower than PCNL group(P<0.05).Conclusion: The overall curative effects of FURL and PCNL were similar in treating proximal ureteral calculi(diameter≥10mm).FURL needs more operation time,but it offers less trama,less postoperative pain and quicker recovery than PCNL,and is worthy of popularization. |
