Acute kidney injury(AKI)is frequently complicated in patients with sepsis.Meanwhile,high incidence and high mortality rate of AKI in sepsis patients are the major difficulties and challenges in the field of the department of critical care medicine.Some progress has been made in the related research,new ideas are continuously emerging,and there are some different voices in this field.In the study,the research progress of sepsis pathogenesis and prevention of AKI is reviewed from the following aspects:1)The incidence of sepsis in AKI is increasing worldwide,combined with high mortality and poor prognosis.2)The understanding of the pathogenesis of AKI in sepsis transfers from simply focus on low perfusion of microcirculation to attach importance to microcirculation.Inflammatory mechanism is complicated.Activation of coagulation cascade,ischemia-reperfusion injury,renal tubular epithelial cell apoptosis,autophagy and other pathological processes play important roles in the development of AKI in sepsis.3)Maintenance of hemodynamic stability should be emphasized primarily in terms of prevention and control to develop individual recovery goals.4)Restoration of immune homeostasis is the hotspot of future research,active control of the source of infection ishelpful to reduce the incidence of AKI and improve the prognosis.5)The application of diuretics,insulin and other drugs and liquids for AKI in sepsis.6)Pay attention to the interaction of lung and kidney on renal function.7)The timing of RRT remain controversial.8)Early identification of AKI in sepsis.
