The Clinical Characteristics Analysis Of Nocardia Culture Positive Cases

Objective To analyze the clinical manifestations of Nocardia culture positive cases and improve the understanding of the bacteria.Methods The clinical data of 20 cases with specimens culture of Nocardia positive from May 2012 to May 2016 in the general Hospital of Ningxia Medical University were studied retrospectively.Results 3 of the 20 cases were suspected contaminated,17 cases were diagnosed infection in different part of the body,among them,2 cases were disseminated infection,12 cases were pulmonary infection,3 cases were soft-tissue infection,1 case was peritoneal dialysis-related peritonitis,17 cases all had underlying diseases,7 of the 17 cases used corticosteroids or immunosuppressants because of underlying diseases.2 of the 17 cases died,the remaining 15 cases improved after treatments with TMP-SMZ,minocycline,amikacin,meropenem,third-generation cephalosporins,etc,and these cases have been followed:2 cases lost follow-up,4 cases recurred of which 3 cases have bronchiectasis combined,9 cases were cured of which 5 cases take oral TMP-SMZ combined with or without minocycline for 6 months to 15 months.Conclusion Nocardia can cause infection of various parts of the body,pulmonary infection is most common.Nocardia infection take place easily on the patient who have underlying diseases,specially who used corticosteroids or immunosuppressants.Nocardiosis has no specific clinical manifestations.Sulfonamides are the first-line antimicrobials to treat nocardiosis,other antimicrobials are also optional like Aminoglycosides,third generation cephalosporins,etc.The prognosis of Nocardiosis associated with infection sites,whether combined with underlying diseases or timely treatment and other factors.
