Objective: To study the effect of neoadjuvant chemotherapy on the degree of sedation of propofol under target-controlled infusion.Methods: 60 patients with modified radical mastectomy were divided into chemotherapy group(group ?)and non-chemotherapy group(group ?),30 per group,according to whether they had neoadjuvant chemotherapy before the operation.Anesthesia induction was performed with a target concentration of 1.5 ug/mL as the initial target-controlled infusion.The concentration was added 0.5 ug/mL each time when the equilibration was reached,until the Bispectral Index(BIS)was lower than 40.At this time,the infusion was terminated.Then the dosage of propofol,the predicted ventricular concentration,mean arterial pressure(MAP)and heart rate(HR)of two groups when BIS reached 40 were recorded respectively.The BIS values of 0.5min,1min,1.5min,2min,2.5 min,3 min,3.5 min and 4 min after infusion and the MAP and HR of when BIS reached 40 and 50 were also recorded.Moreover,the time when BIS was less than 40 and between 40 and 60 was wrote down.The number of patients ephedrine and atropine used in the whole process was also recorded.Results: It was not statistically significant to compare the differences in age,body mass index(BMI),basal BIS value,mean arterial pressure(MAP)and heart rate(HR)between the two groups(P>0.05).When BIS amounted to 40,the differences in the usage of propofol and predicted ventricular concentration between the two groups were found to be statistically significant(P<0.05);but that in MAP and HR were not(P>0.05).After the infusion,the BIS values of the patients in chemotherapy group at 1.5min,2min,3min,3.5min and 4min were lower than that in untreated group(P<0.05);the comparison of the BIS value between the chemotherapy group and the non-chemotherapy group was meaningless statistically at 0.5min,1min and 2.5min(P>0.05);There was no significant difference in MAP and HR between the two groups when BIS amounted to 50 and 60(P>0.05).The time when BIS was lower than 40 in chemotherapy group was longer than that in the non-chemotherapy group,and it was meaningless to compare the time when BIS was less than 60 in two groups(P>0.05).The number of patients used ephedrine and atropine in the chemotherapy group was higher than that in the untreated group.Conclusion: It can be found that neoadjuvant chemotherapy is easier to achieve deep sedation of propofol under target-controlled infusion in patients with breast cancer,and can also prolong the effect of propofol anesthesia in these patients when depth of sedation.
