| Objective: To observe the analgesic effects,drug dosage and side effect occurrence of patient controlled analgesia without background infusion in cesarean section patients.Method: 30 healthy pregnant women(ASA I ~ II),who were suffered from cesarean section under spinal anesthesia,were randomly divided into the following two groups: control group(10 minutes lockout time,0.5ml bolus dose and 2 ml/h background infusion rate)and experimental group(10 minutes lockout time and 1ml bolus dose without background infusion).Pentazocine150 mg and fentanyl 0.5mg in 100 ml normal saline were administrated according to above protocol in each group.Pentazocine 30 mg and tropisetron5 mg were administrated 15 minutes before the end of surgery.Epidural catheter was extracted before the patients were sent back to the ward with PCIA pump.Postoperative VAS scoring and Ramsay scoring were observed in each group at 4h,8h,12 h,24h after surgery.Other indexes,such as total drug dosage were observed at the same time point.Duration of the operation and amount of bleeding were recorded.The incidences of side effects,such as bradycardia,hypotension,respiratory depression,nausea,vomiting and urinary retention were also recorded.Result: There were no significant differences in VAS scoring and Ramsay scoring between the two groups at all time points.There were no significant differences in the incidences of bradycardia,hypotension,respiratory depression and vomiting between the two groups.The dosage in experimental group was significant decreased at 4h,8h and 24 h after surgery compared with that in control group(P<0.05).The experimental group,s dosage of 8-12 h was significant higher than control group,s(P<0.05).The incidence of nausea in control group(33.3%)was significant increasedcompared with that in experimental group(26.7%).Conclusion: PCA without background infusion plays analgesic and sedative role in cesarean section patients with decreased dosage and lower nausea incidence. |
