Objective: To investigate the prevalence of Genitourinary syndrome of menopause and pelvic floor disorders among middle-aged women in Beijing community and to assess the relevant factor.Methods: To investigate the women in 35-70 years old from October2014-December 2014 in jurisdiction of Yuetan Community Health Service Center of Fu Xing Hospital,Capital Medical University and The Second Hospital of Beijing.A questionnaire included general situation,education background,occupation,monthly income,body mass index(BMI),menstruation,delivery mode and gynecological surgery.The PFD was investigated using pelvic floor distress inventory-short form 20(PFDI-20).The women who participated in the study were all examined by gynecological examination to determined whether they had pelvic organ prolapse(POP).Respondents were voluntarily participated and signed informed consent.The data were input into the spss19.0 for statistic analysis,logistic regress analysis was adopted to analyze the relevant factors of PFD and POP.Results:1 General situation: A total of 3000 people was investigated,2791 questionnaires was effective;effective response rate was 93%.The average age was(53.61±7.06)years,the average menopause age was(49.92±3.61)years.2 The prevalence of vaginitis atrophic: the prevalence of the vaginitis atrophic was 31.351%(875/2791),95%CI was0.2963-0.3307.The prevalence of vaginal pain was 8.205%(229/2791);the prevalence of pain during intercourse was 15.514%(433/2791);the prevalence of vaginitis itching was 19.527%(545/2791);the prevalence of burning was 7.847%(219/2791).3 The relations between vaginitis atrophic and menopause: the prevalence of the vaginitis atrophic of women in perimenopause was 3.296%(92/2791);the prevalence of the vaginitis atrophic of women in postmenopause was19.025%(531/2791).The prevalence of the vaginitis atrophic in postmenopause was greater than that of women in perimenopause.4 The prevalence of PFD: the prevalence of PFD was 64.314%(1795/2791),95%CI was 0.3391-0.3746.5 The relations between PDF and delivery mode: the prevalence of PDF of women in vaginal delivery was 67.455%(1283/1902);the prevalence of PDF of women in cesarean section was 58.309%(400/686).The prevalence of the PDF in vaginal delivery was greater than that of women in cesarean section(X2 18.546,P<0.05).6 The relevant factors of PDF: perimenopause OR: 2.266 95% CI1.173-4.379;physical disease OR: 2.142 95%CI 1.456-3.151;taking medicine OR: 2.711 95%CI 1.668-4.405;secondary education OR: 0.435 95%CI0.271-0.699;vaginitis atrophic OR: 2.701 95%CI 1.743-4.186.7 The relations between POP and delivery mode: the prevalence of uterine prolapse,anterior vaginal wall prolapse,posterior vaginal wall prolapse were 2.735%(75/2742),11.233%(308/2742)and 4.850%(133/2742),respectively.The prevalence of vaginal delivery and cesarean section were14.507%(272/1875)and 9.3%(63/674),respectively.Vaginal delivery and cesarean section had statistical significance(X2 11.562 P<0.05).8 The relevant factors of POP: If the odds ratio(OR)were taken as 1.0for the POP women who in 35-44 years,then OR for 45-54 years and55-64 years would be 2.601 and 2.862,respectively;if the odds ratio(OR)were taken as 1.0 for the POP women who in normal weight,then OR for overweight and obesity would be 1.725 and 3.612,respectively;Similarly,the OR for vaginal delivery was 2.342.The women who underwent gynecological surgery and gynecological diseases,the OR were 2.458 and 3.350,respectively.Conclusions:1 The prevalence of vaginitis atrophic is related to menopause;2 Menstruation,delivery time,physical disease,taking medicine,educational background and vaginitis atrophic are relevant factors of PFD;3 Age,BMI,delivery mode,gynecological surgery,gynecological diseases and educational background are relevant factors of POP.
