Objective: Henoch-Schonlein Purpura(HSP)is a vessels inflammatory diseases,major in involvement systemic small blood vessels.Scope of lesions involving skin,gastrointestinal tract,joints,kidneys and other organs.Its incidence in children showed a trend of rising year by year.When kidney damage happened,called Henoch-Schonlein Purpura Nephritis(HSPN)is a serious complications in HSP and it's the most common secondary kidney disease in children.However,the exact pathogenesis remains unclear.It is generally believbd that the cause of HSP may for a variety of infections,acting on the genetic background of the individual,led to the immune system disorder.High mobility group protein 1(HMGB1)as a medium of inflammation is gradually been recognized,Its plays an important role in the expansion and maintain the inflammatory response,in endothelial cell function and immune adjustment.This experiment by detecting serum HMGB1 levels to explore the relationship between HMGB1 and HSP acute phase.And study its whether there is any difference in different clinical manifestations of HSP.In order to further search for the cause ? condition assessment and targeted therapy provide new ideas and new methodsMethod:1 Collect data and measurementsChoose patients diagnosed with HSP in Hebei medical university second hospital pediatric kidney professional ward between December 2015 and August 2016.65 cases of total,Type of joint type purpura in 15 cases,abdominal purpura 15 cases,skin purpura type 16 cases,19 cases purpura nephritis.At the same time from 15 cases in our hospital outpatient medical normal children as control group.Test all the subjects of HMGB1 content in serum samples and do statistical analysis.And detection the correlationanalysis of HMGB1 with white blood cells,platelets,IL-4,urea,creatinine,urinary inhibition,beta 2 C microglobulin,IgA,C3,C4 and so on.2 Subsistence of sampleGathering healthy controls and HSPchildren venous blood 2 ml before treatment.To return after centrifugal supernatant,save into-80?cryogenic refrigerator.With double antibody sandwich solid-phase method of enzyme-linked immunoso-rbent(ELISA)to detect the contents of serum HMGB1.3 Statistical analysisApplication SPSS21.0 statistical software for statistical description and statistical analysis.When the data conform to the normal distribution describe with mean,standard deviation,When the data does not conform to the normal distribution describe with the median,interquartile range.Using two sample t or t?test,Nonparametric analysis comparison between the two groups use the Mann-Whitney test.P < 0.05 is considered statistically significant.Correlation analysis using the Pearson or Spearman rank correlation.Result:1 All the 65 cases has 31 boys and 34 girs,Detection of serum level of HMGB1 in chilren with different gender,boys:3805.20(2056.46)pg/mL.girs:3590.07(1841.64)pg/mL,Z=-5.83,P>0.05,there was no statistical significance2 In the normal control group and HSP group serum level of HMGB1respectively:2012.48±851.65pg/mL,3551.76±1258.14pg/mL,Two groups of comparison t?=-5.71,P<0.01 is of significance in statistics science.3 In the normal control group,HSPN group and NHSP group the serum level of HMGB1 detection results respectively:2012.48±851.65pg/ mL,3235.49±1196.56 pg/mL,4317.48±1083.59pg/mL,Three groups compared F=17.94,P<0.01 is of significance in statistics science.Comparing the two results:Normal control group compared with NHSP groupP<0.01 is of significance in statistics science.Normal control group compared with HSPN groupP<0.01 is of significance in statistics science.HSPN group comparedwith NHSP groupP<0.01 is of significance in statistics science.4 In the group of joint type purpura,abdominal type purpura,skin type purpura and purpura nephritis group the serum level of HMGB1 detection results respectively: 3314.15±1146.35pg/mL,3371.41±1303.90pg/mL,3034.33±1189.16pg/mL,4317.48±1083.59 pg/ mL.Four groups com-pared: F=4.03,P<0.01 is of significance in statistics science.Compared with the two groups:Joint type purpura and abdominal type purpura group P>0.05 there was no statistical significance.Joint type purpura with skin type purpura P>0.05 there was no statistical significance.Joint type purpura with purpura nephritis group P<0.05 is of significance in statistics science.Abdominal type purpura with skin type purpura P>0.05 there was no statistical significance.Abdominal type purpura with purpura nephritis group P<0.05 is of significance in statistics science.Skin type purpura with purpura nephritis group P<0.01 is of significance in statistics science.5 Serum level of HMGB1 is positive related to the white blood cell count,The relevant index was 0.31,P<0.05 is of significance in statistics science.6 Serum level of HMGB1 is positive related to the Ig A,The relevant index was 0.44,P<0.05 is of significance in statistics science.7 Serum level of HMGB1 is positive related to the Serum creatinine,The relevant index was 0.55,P<0.05 is of significance in statistics science.Conclusion:1 The serum level of HMGB1 has no statistical significance in different gender2 In HSP the serum level of HMGB1 expression quantity is higher than in normal control group,And HSPN group is higher than the NHSP group,Indicate that HMGB1 participart in the pathogenesis of HSP and its occurrence of kidney damage.3 The serum level of HMGB1 in abdominal type purpura,joint type purpura,purpura nephritis groups are higher than in the skin purpura,shows that HMGB1 reflects the organ damage and disease severity of HSP in some estent?4 The serum level of HMGB1 is positive related to the white blood cell count?IgA and the serum creatinine,Speculated that HMGB1 may induce the course of the inflammatory response and immune regulation process in HSP,And closely associated with kidney damage.
