Objective: To explore the relationship between gynecological diseases and body composition change,the human body composition was analyzed in patients undergoing gynecological surgery by using bioelectrical impedance method.Combined with dietary survey,nutritional risk screening and detection of biochemical indexes to assess their nutritional status,to provide basis for safe operation support and rehabilitation plan.Methods:1 According to the inclusion criteria,142 cases of gynecological surgery were included as case group from January 2016 to April 2017 in the first hospital of Hebei Medical University.At the same time,59 healthy women were selected as control group from the first hospital of Hebei Medical University examination center.2 We collected general information,age,height,weight,and calculated body mass index(Body mass index,BMI)of objects included in the study;measured the resting metabolic rate,body fat mass,body fat percentage,visceral fat area,muscle mass,protein content,water content,intracellular fluid volume,extracellular fluid volume,mid arm circumference,presumed bone mass using the Inbody 720 body composition analyzer.3 Dietary survey was conducted,including whether or not to receive diet education,medical history,whether or not vegetarian,whether the timing of quantitative,cooking oil,eating habits,whether or not to eat sweets,exercise,etc..Blood biochemical indexes were detected,including potassium,sodium,urea,creatinine,uric acid,ALT,AST,total bilirubin,blood glucose,triglyceride,total cholesterol,total protein,albumin,hemoglobin,white blood cell count,lymphocyte count,C-reactive protein,neutrophils.4 Input all data into excel software to establish database.Statisticalanalysis was conducted using statistical software SPSS 16.0.For quantitative variable,consistented with normality and homogeneity of variance,make the statistical description using the((?)±s),two groups were compared with T-test,multiple groups were compared using ANOVA.While the non-normally data,M(QR)is used to describe,nonparametric test was used for statistical analysis.Categorical variables were expressed by frequency,rate or proportion,Chi-square test was used for statistical analysis.All statistical analysis was performed using a two-sided test,the ? =0.05 as the standard test,P value equal to or less than 0.05 was considered statistically significant.Results:1 In this study,a total of 142 patients with gynecologic diseases(case group)and 59 normal women(control group)were studied.There are 45 patients with less than 40 years old and 97 patients older than or equal to 40 years old in the case group,under the age of 40 was 21 and more than 40 was 38 in the control group,these differences in age were not statistically significant.Cases included 16 patients with ovarian cysts,13 patients with ovarian tumor,55 patients with hysteromyoma,25 patients with endometrial polyp,8 patients with endometrial carcinoma,5 patients with endometrial lesion,and 20 other gynecological diseases.2 Cases and control group analysis of body compositionThe case group exhibited higher levels BMI,mid arm circumference and arm muscle circumference than did the control group(P<0.05);Compared with the control group,there were significantly difference in fat content,body fat percentage and visceral fat area(P<0.05);There were no significantly difference in the non fat content,protein content,water content,intracellular fluid and extracellular fluid between the case group and the control group(P > 0.05);Compared with the control group,there was no significant difference in resting metabolic rate(P > 0.05).3 BMI method and body fat rate method to evaluate the level of constitution in patients with gynecological diseases3.1 BMI method to evaluate the constitution level of patients with gynecological diseasesAccording to the BMI evaluation method,there were 2 patients with low body weight(accounting for 1.41%),and 42 patients with normal weight(accounting for 29.58%),50 with overweight(accounting for 35.21%)and 48 with obesity(accounting for 33.80%).3.2 Body fat rate method to evaluate the constitution level of patients with gynecological diseasesAccording to the evaluation of the body fat rate method,all patients,underweight was accounted for 0.71%(1/142),normal weight accounted for5.63%(8/142),overweight accounted for 15.49%(22/142)and obesity accounted for 78.17%(111/142).The Fisher exact probability analysis,P=5.25*10-20,the difference was statistically significant.There were differences between the two methods.4 Analysis of eating habits and exercise habits of patients with gynecological diseasesFor 142 cases of gynecological diseases,people who like to eat sweets up to 71.13%(101/142),and accounted for 78.87%(112/142)patients said they usually do not have the habit of exercise.5 Comparison of resting metabolic rate in patients with different body fat percentageAccording to the different body fat rate,patients were divided into three groups.There were statistically significant difference between the three groups:(F=3.853,P=0.024)with normal group(1265.25±74.98),overweight group(1221.27±99.081),obesity group(1286.39±102.65).After multiple comparisons,the results showed that there was significantly difference in the resting metabolic rate between the patients with overweight and obesity,and there was no significant difference between the other two groups.6 Comparison of biochemical indexes in patients with different body fat percentage6.1 Comparison of blood glucose and blood lipid in patients with differentbody fat percentageThere were significantly differences in triglycerides among different groups(normal,overweight and obesity)of patients with gynecological diseases(?~2=11.756,P=0.003).With the increase of body fat rate,triglyceride level showed a rising trend.Nevertheless,the blood glucose(c 2=2.594,P=0.273),total cholesterol(?~2=0.240,P=0.887)still can not believe that there were differences.6.2 Comparison of electrolytes,liver and renal function in patients with different body fat percentageDifferent body fat rate in patients with gynecologic diseases group kalium(?~2=0.001,P=0.999),sodium(?~2=1.276,P=0.528),urea(?~2=3.635,P=0.162),creatinine(?~2=3.646,P=0.162),uric acid(?~2=3.468,P=0.177),AST(c 2=2.414,P=0.299),total bilirubin(c 2=2.094,P=0.351)were not different.With regard to ALT,there was statistical difference between the three groups(?~2=9.065,P=0.011).7 Comparison of nutritional indexes in patients with different body fat percentageThe total protein(F=0.326,P=0.999)and albumin(F=0.405,P=0.668)in patients with different body fat percentage were not statistically different,and all were within the normal range.Compared with the three groups,the hemoglobin content(F=9.843,P=0.007)was statistically significant.8 Different body fat rate of patients with past medical historySpearman rank correlation analysis showed that the body fat rate of patients with gynecological diseases was significantly correlated with past medical history,the correlation coefficient was 0.326,P<0.05,the difference was statistically significant.Conclusion:1 The rate of fat,body fat and visceral fat area in patients with gynecological diseases were significantly higher than those in normal subjects.However,the non fat content,muscle content and water content,as well as the estimated bone mass are no different from those of normal people.2 Most people like sweets and lack of physical exercise.These unhealthy lifestyles may lead to a marked increase in body fat.To provide effective guidance and intervention for overweight,obesity women may prevent the occurrence of gynecological diseases to some extent.3 There was no gender difference in the BMI method for the evaluation of body weight,and the current BMI standard had an impact on the results of physical evaluation in patients with gynecological diseases.It suggested that the BMI should be changed.4 With the increase of body fat rate,the level of triglyceride and ALT in patients with gynecological diseases were increased.The body composition analysis of gynecological disease patients,targeted for fat loss is of great significance.Body composition analysis should be routine examination,targeted to reduce body fat is of great significance.5 It is reasonable to classify body composition analysis as a part of the physical examination,which can provide more guidance and help for the detection,diagnosis,prediction and treatment of gynecological diseases and other diseases.
