Objective: To explore the lymph vessel tumor emboli grading system and prognosis of invasive invasive carcinomas of no special type(ICNST)of the breast.Methods: 466 cases of ICNST from the Fourth Hospital of Hebei Medical University form January 2006 to December 2008 were selected.Clinical data were analyzed.The expression of D2-40,ER,PR and HER2 were analyzed by immunohistochemistry.The number of lymph vessel tumor emboli were defined.Grades for lymph vessel tumor emboli on the number of mitotic and apoptotic figures in the tumor cells in one high-power field were classified.The overall survival was followed up by telephone.Correlation analysis was performed using Spearman test.Kaplan-meier methods were used to analyze the survival rate.Log-rank tests were used to analyze the survival curves.Multivariate Cox proportional hazard model was used to analyze influence factors of survival time.Results: Of the 466 cases,280 cases were grade 0(60.09%);112 cases were grade 1(24.03%);58 cases were grade 2(12.45%);16 cases were grade3(3.43%).Correlation analysis results showed that lymph vessel tumor emboli grading system was positively correlated with the number of lymph node metastasis(P<0.001).Kaplan-Meier single factor analysis showed that histological grading,lymph vessel tumor emboli grading system,the number of lymph node metastasis,the expression of ER,PR and HER2 and Molecular typing were related to the survival time of patients(P<0.05).Multivariate analysis of Cox proportional hazard model showed that lymph vessel tumor emboli grading system and the number of lymph node metastasis were independent prognostic factor in patients with ICNST(P<0.05).Conclusion: The grading system for lymph vessel tumor emboli couldpredicte the outcome of patients with ICNST,the pathologist should pay attention to this feature.
