Objective:To analysis the species of pathogens causing infections in burn patients and their resistance to commonly used antibiotics in our hospital so as to guide the clinical medication.Methods: 888 specimens of pathogens in burn patients were collected from burn surgical department of Shi Jiazhuang Hebei Youai Hospital from Jan 2010 to Dec 2014.The bacteria was identified by using automatic microbal identification.The K-B disk-diffusion method was used for drug-sensitive test.The statistics of the distribution trend and drug resistance of pathogens causing infections were collected.Results:1 From 2010 to 2014,the positive rate of samples was about 30%.The top five detected pathogen was Pseudomonas aeruginosa(36.71%),Staphylococcus aureus(22.86%),Escherichia coli(14.86%),Klebsiella pneumoniae(8.22%)and Acinetobacter baumannii(6.31%).2 The detection rate of Pseudomonas aeruginosa was high.The detection rate of Staphylococcus aureus had slightly deceased in 2013(P<0.05).The detection rate of MRSA had increased year by year(P<0.01)and the detection rate of MSSA had no satistics value in 5 years.The detection rate of Escherichia coli had deceased in 2010(P<0.05).The detection rate of Klebsiella pneumoniae had increased in 2014(P<0.01)and was already higher than that of Acinetobacter baumannii.3 Since 2012,the resistance of Pseudomonas aeruginosa had deceased year by year.The drug resistance rate of Pseudomonas aeruginosa to ciprofloxacin and piperacillin-tazobactam had inceased year by year.Acinetobacter baumannii had higher drug resistance rate to cephalosporins,carbapenems and ciprofloxacin,including piperacillin,piperacillin-tazobactam,meropenem and ciprofloxacin.The drug susceptibility rates to otherantibiltics were reduced.More than 80% of the Escherichia coli strains were resistant to ampicillin,cefazolin and gentamicin.The drug resistance to other drugs were lower,while it increased year by year.The drug resistance of Klebsiella pneumoniae was lower than other pathogens' except the second generation cephalosporins.Klebsiella pneumoniae has more sensitive to the common antibiotics.4 The detection rate of MRSA had increased year by year(P<0.01).The 100% resistance rate of MRSA in 2010-2014 had 3 antibiotics including penicillinG,oxacillin,trimethoprim/sulfamethoxazole.The resistance rate over 80% had 8 speices,while the resistance rate of MRSA to ciprofloxacin,levofloxacin,gentamicin had decreased year by year.No vancomycin,linezolid resistant strain was detected.The change of rate of resistance in MSSA had no satistics value in 5 years.More than 80% of the MSSA strains were resistantto penicillin G and trimethoprim/ sulfamethoxazole.Less than 20% of the rate resistance had 6 speices antibiotics including oxacillin,gentamicin,ciprofloxacin,levofloxacin,vancomycin and linezolid.Conclusion: The pathogens causing infections in burn patients in our hospital are mainly caused by Pseudomonas aeruginosa and Staphylococcus aureus.According to the results of drug sensitive test and the distribution trend in 5 years the correct drug choice for treatment have great significance to reduce the occurrence of bacterial drug resistance.
