Non Traumatic Osteonecrosis Of The Femoral Head Injury Intervention In Longzhong Sunshang Capsule Clinical Research

Objective:To observe and evaluate the clinical effect and safety of the treatment of nontraumatic necrosis of the femoral head with the combination of Longzhong Sunshang Capsule combined with core decompression and bone grafting,and to explore the therapeutic effect of "promoting blood circulation therap" of traditional Chinese medicine in the treatment of nontraumatic necrosis of the femoral head.Method:Data of 125 patients of non-traumatic necrosis of the femoral head in accordance with the inclusion criteria from February 2015 to December 2016 from Departments of Orthopaedics and Traumatology and Joint,Gansu Province Hospital of TCM were choosen,and 114 cases completed the final test.They were randomly divided into treatment group(Longzhong Sunshang Capsule combined with core decompression and bone grafting)57 cases and control group(core decompression and bone grafting)57 cases.The treatment group was treated with core decompression and bone grafting and Longzhong Sunshang Capsule was given to the postoperative patients,4 to 6 grains(0.5g)each time,two times a day,30 days as a course of treatment,continuous taking 3 course of treatment;the control group was only treated with core decompression and bone grafting.3 months of observation was given in the preoperative and postoperative about the changes of VAS score,Harris hip score,D-Dimer,prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinoge(Fbg),thrombin time(TT).A follow-up was given after 6 months of the postoperative for the changes of hip Harris score and clinical efficacy.Result:1.General data comparison: there was no significant difference between the two groups in age,gender,type and location of the disease,but this study found that the right side of the disease more than the left side.There was no significant difference between pre and post treatment(P>0.05).2.VAS score: The two groups were compared between pre and post treatment,the VAS score differences were statistically significant(P<0.05);when VAS score was compared with after treatment,the control group(3.98±1.87)was higher than the treatment group(3.07±1.70),the difference was statistically significant(P=0.008<0.05).3.Harris hip score:When the two groups were compared between pre and post treatment,the Harris score differences were statistically significant(P<0.05);the Harris score(79.70±6.94)of the treatment group was significantly better than the control group(76.15±9.66)after treatment,the difference was statistically significant(P=0.026<0.05);after 6 months,treatment group the Harris score(81.50±5.52)was significantly better than the control group(77.88±9.45),the difference was statistically significant(t=2.502,P=0.014<0.05).4.Coagulation related indicators:When the indexes of PT,APTT,Fbg,TT and D-Dimer were compared,the difference of the treatment group between pre and post treatment was statistically significant(P<0.05);when Fbg and TT were compared in the treatment group between pre and post treatment,the difference was statistically significant(P<0.05);compared with the control group,PT,APTT,and TT were prolonged obviously,Fbg and D-Dimer were decreased,the difference was statistically significant(P<0.05).5.When the clinical efficacy of the two groups was compared after the treatment,the effective rate of the treatment group was 94.74%,the effective rate of the control group was 82.46%,the difference was statistically significant(Z=2.305,P=0.021<0.05).Conclusion:A observation after 3 months and 6 months of the postoperative shows the treatment of Longzhong Sunshang Capsule combined with core decompression and bone grafting in non-traumatic necrosis of the femoral head can significantly improve the clinical effect of treatment,improve the patient's hip joint function and quality of life,and no adverse reactions and side effects were found.In promoting blood circulation;Longzhong Sunshang Capsule composing prescriptions by "promoting blood circulation therapy" of traditional Chinese medicine in the treatment of non-traumatic necrosis of the femoral head can improve the coagulation function of patients,promote blood circulation,and play an important role in the treatment of femoral head necrosis.
