The Predictive Value Of PIRO Score Combined With Serum Procalcitonin In Sepsis Prognosis Evaluation

Objective: To investigate the predictive value of PIRO score combined with serum PCT in sepsis prognosis evaluation.Method: A total of 128 sepsis patients admitted to emergency department in affiliated hospital of Zunyi Medical College(2014.01-2016.01)were recruited in this study.General information,basic vital signs of all subjects as well as their WBC,s PCT,CRP levels within 24 h were collected for PIRO score calculation.According to the disease severity,all cases were divided in to sepsis group and severe sepsis group(including septic shock),and were divided into survival group and deceased group based on the survial condition of28 d.Then ROC was applied to analyze PIRO scores and procalcitonin in order to evaluate the abilities in predicting sepsis prognosis.Result: 1.The overall mortality of 128 cases was 18.8%;2.The PIRO score,age and PCT level in deceased group were higher than those in survival group(p<0.05),while no significant difference was found in CRP,WBC and sex ratio between two groups(p>0.05);3.CRP,s PCT and PIRO score in severe sepsis group were higher than those in sepsis group(p<0.05),while no significant difference in WBC,basic vital sign and hospital stay was found(p>0.05);4.By Logistic regression analysis,s PCT and PIRO score were the independent risk factor for evaluating disease severity(OR=1.505,1.350,p<0.05)and 28 d prognosis(OR=1.096,1.465,p<0.05);5.By ROC analysis of disease severity,the AUC(0.859),specificity(91.8%),positive predictive value(82.3%),negative predictive value(84.1%)and Youden index(58.6%)of PIRO score combined with serum PCT was superior to those indicators PIRO score alone(0.798;83.5%;71.5%;78.8% 40.6%).By ROC analysis of 28 d prognosis,the AUC(0.831),specificity(89.5%),positive predictive value(76.7%),negative predictive value(87.6%)and Youden index(60.2%)of PIRO score combined with serum PCT was superior to those indicators PIRO score(62.7;81.2%;56.5%;87.6% 44.1%).Conclusion: 1.PIRO score and s PCT were the independent risk factors for evaluating sepsis severity and 28 d prognosis;2.The combination of PIRO score and s PCT had superior predictive value in sepsis prognosis evaluation to the application of PIRO score alone.
