Objective:Antipsychotic induced metabolic abnormalites not only brings bad subjective experience for the patient,but also lead to lipid metabolism disorder and increased risk of type 2 diabetes,seriously affecting the quality of life of the patients,increase the risk of recurrence.Therefore,the research for the relationship between COMT(rs4680),MTHFR(rs1801133)gene polymorphism and metabolic abnormalities is the purpose of this research.Methods:According to the group exclusion criteria included 355 patients,all patients received antipsychotic treatment.Do observation on 12 weeks,body weight and waist circumference were measured periodically,further calculation of body mass index(BMI),venous blood and the relevant indicators;using high salt extraction method to extract DNA and with the method of MALDI-TOF-MS to genotype the COMT(rs4680)and MTHFR(rs1801133).Statistical analysis of the data using SPSS 20 software package.Results:1.The weight gain was observed in 155 cases,without weight gain there were 200 cases,visible in the age difference was statistically significant(t= 3.254,P<0.05).The first 88 cases(24.8%),the body weight increased in 48 cases(31%),40 cases without increasing weight(20%),there was significant difference between the two groups(?2= 5.634,P<0.05).The occurrence of metabolic syndrome were 126 cases,two cases and the first difference in gender was not statistically significant(P >0.05),but in the age distribution,visible Met S age is 41.02 ± 11.27 years,without Met S the age is 37.55± 11.38 years,there was a significant difference between the two groups(P<0.05).2.In the baseline level,between weight gain group and without weight gain group the index of FPG,T3 and T4 had significant difference(P<0.05).Compared to the without weight gain group the weight gain group after treatment with single antipchchotic,at 8 and 12 weeks,the body weight,BMI and waist circumference were significantly increase(P<0.05),TG,LDL and CHOL were significantly increased(P<0.05).3.In the comparison of different antipsychotic drugs,the of olanzapine group gained the most highest weight gain,followed by the quetiapine group,the risperidone group.The weight,waist circumference,BMI,LDL and CHOL of the three groups were statistically significant(P<0.05).4.COMT(rs4680)genotype and allele frequency in weight gain and without weight gain,the distribution differences between groups were not statistically significant(P>0.05),distribution in males and females were not statistically significant(P>0.05);Allele frequency in occurrence of Mets,the distribution differences between groups were not statistically significant(P>0.05),distribution in males and females were not statistically significant(P>0.05).5.The results showed that MTHFR(rs1801133)GG,GA,AA have significant differences in the indexes of ?LDL and ?CHOL.6.MTHFR(rs1801133)genotype and allele frequency in weight gain and without weight gain,the distribution differences between groups were not statistically significant(P>0.05),distribution in males and females were not statistically significant(P>0.05);Allele frequency in occurrence of Mets,the distribution differences between groups were not statistically significant(P>0.05),distribution in males and females were not statistically significant(P>0.05).Conclusions:1.SGAs drugs can cause significant weight gain,the first episode schizophrenia patients and younger patients are tend to get more weight gain.2.There was a significant difference in metabolic indexes between the weight gain group and the without weight gain group.3.There were significant differences in body weight changes caused by different SGAs,and the weight gain in olanzapine group get the highest weight gain,followed by quetiapine group,and finally risperidone group.4.COMT(rs4680)is not associated with metabolic abnormalities caused by antipsychotic drugs.5.The difference in LDL and CHOL between the different genotypes of MTHFR(rs1801133)after taking medicine was statistically significant,indicating that GG genotypes may be related to metabolism of LDL and CHOL.
