Aim Liver cirrhosis is one of the most common human digestive diseases,which is a chronic progressive liver disease owing to various liver injuries,mainly including hepatitis virus infection in China,following excessive alcohol consumption,autoimmune liver disease,etc.There are varied complications such as upper gastrointestinal bleeding,ascites,hepatic encephalopathy and infections for advance liver cirrhosis,which is decompensated cirrhosis with poor prognosis.Hepatocellular carcinoma(HCC)is one of common malignant tumors with a high mortality in China and a worldwide public health problem,and the majority of HCC occurs on the basis of liver cirrhosis.The liver connects with the gut through “gut-liver axis”,and the quanitity,compositon and distribution of intestinal flora and liver influence each other so much.The intestinal microbiota of patients with liver diseases can be disordered,and the occurrence and prognosis of liver diseases can be implicated in the quantity and distribution of gut microbiota conversely.Over the past decade,there are increasing researches characterizing the gut flora of patients with liver cirrhosis but not HCC.In this paper,the fecal microbial communities of liver cirrhosis and HCC with liver cirrhosis patients were analyzed by 454 pyrosequencing of the 16 S r DNA.Content The aim of this paper was to compare and analyze the difference and characteristics between liver cirrhosis patients and HCC patients with liver cirrhosis,and to investigate the relations between HCC with liver cirrhosis and the composition and structure of intestinal flora preliminarily.Methods Between December 2015 and May 2016,a total of 35 patients with chronic liver disease hospitalized at the Department of Hepatology,Tianjin Third Central Hospital were enrolled and divided into two groups: 20 liver cirrhosis patients as the cirrhosis group and 15 HCC patients with live cirrhosis as the HCC group.35 fecal samples were collected,total bacterial DNAs were extracted and submitted the high throughout 16 S r DNA sequencing on the V3-V6 viable region by using Roche 454 (GS FLX Titanium System).The 16 S r DNA tags and operational taxonomic units(OTU)were then obtained from the sequences.The bioinformatic analysis of taxonomy,abundance and alpha diversity were performed to investigate the changes of intestinal flora in HCC patients with liver cirrhosis.Results The proportions of gut microbiota between liver cirrhosis and HCC were significantly different by the bioinformatic analysis.The proportions of Actinobacteria and Bifidobacterium in cirrhosis group were significantly higher than HCC group in Actinobacteria bacteria.In Bacteroidetes bacteria,the proportions of Rikenellaceae in cirrhosis group were significantly lower than HCC group.The proportions of Clostridium in cirrhosis group was significantly higher than HCC group,while the proportions of Christenellaceae in cirrhosis group were significantly lower than HCC group in Firmicutes bacteria.Conclusion There was significant difference of composition in intestinal flora between liver cirrhosis patients and HCC patients with liver cirrhosis.In our study,the difference and characteristics of intestinal microbiota between liver cirrhosis patients and HCC patients with liver cirrhosis were preliminarily analyzed,and there are many issues that require further study and discussion.
