Objective To determine the express level of Thymidine Kinase 1(TK-1)in fine-needle aspiration washout of thyroid nodules and investigate the significance of washout TK1 level in differentiation between benign and malignant thyroid nodule.Methods In ex vivo group,samples were collected form 103 nodules during operation in Tianjin Medical University General Hospital from Jan May to Dec 2016,of which final pathological diagnosis were 64 malignant nodules and 39 benign nodules.And the postoperative paraffin pathology confirmed thyroid papillary carcinoma according to age,sex,nodule size,number of lesions,the situation of the film,the surrounding soft tissue,the number of lymph node metastasis,lateral cervical lymph nodes were grouped and been statistical analysis.In vivo group,samples were collected from 42 patients with fine-needle aspiration cytology examination in our hospital at the same time period,of which biopsy confirmed benign or malignant nodules(malignant and suspicious malignant).TK1 levels were measured and clinical data were reviewed to invest its statistical significance.Results In ex vivo group,the mean concentration of TK1 in the fine-need aspiration washout of benign thyroid nodules was(5.973 ± 0.8779)pmol/L,and that of malignant nodules was(8.852 ± 0.5934)pmol/L,the difference was statistically significant.The level of TK1 in the nodular goiter was(2.696 ± 0.2054)pmol/L and thyroid adenoma was(11.22 ± 1.462)pmol/L,the difference between the two was statistically significant.The study of thyroid adenoma(11.22 ± 1.462)pmol/ Land thyroid cancer(8.852 ± 0.5934)pmol/L,TK1 level was not statistically significant.In nodular goiter and thyroid nodules,we found 4.225 pmol/L as the cut-off value for the identification the sensitivity and specificity were 95.83% and 84.38%.The area under the ROC curve was 0.941.There was no significant difference in TK1 level between the age,sex,nodule size,number of lesions and membranous condition group,and there were statistically difference between the surrounding soft tissue,the number of lymph node metastasis and the lateral lymph node group.P value was less than 0.001.In vivo group,the mean concentration of TK1 in the fine-need aspiration washout of benign nodules was(3.72 ± 0.26)pmol/L,and that of malignant nodules was(7.67 ± 0.33)pmol/L,the difference was statistically significant.With the cutoff value of 4.732 pmol/L,the area under the curve was 0.72,and the sensitivity and specificity were 75.61% and 66.29%,respectively.Conclusions The TK1 levels in fine-needle aspiration washout is helpful in differentiation benign and malignant nodules,which is of significance as an adjuvant for thyroid fine-needle aspiration.
