Objective: To know about children's overweight and obesity situation in Dagang oilfield area of Tianjin;to explore the relations of children's overweight and obesity to hyperlipidemia,hyperuricemia and type II diabetes;and to investigate the relations of IR to the levels of LEP,APN and FGF-21 among children of different BMIs.Method:A questionnaire survey with 12476 cases,which includes recording gender,age as well as measuring height(m)and weight(kg),and calculating BMI,was done covering all the middle and primary school students whose ages were between 7-16 in Dagang oilfield area of Tianjin.Excluding those objects whose medical histories including glucose metabolic diseases like diabetes,hyperthyroidism,hypothyroidism,apituitarism,and Cushing's syndrome,etc.and who have major stressful events recently,876 children(478 males,398 females)were approved by their parents signing informed consent form to accept clinic blood biochemical index test.We diagnosed overweight and obesity children according to Body Mass Index Reference Norm for Screening Overweight and Obesity in Chinese Children and Adolescents.876 objects were divided into overweight group,obesity group and normal group,and we compared the fasting blood glucose,uric acid,blood fat in between these three groups.Taking gender and age into consideration,we randomly selected 30 cases(male: female=1:1)from each group,testing their fasting blood glucose,insulin and serum LEP,APN,FGF-21,and calculated HOMA-IR,and then analyzed the relations of LEP,APN,FGF-21 to IR.Results: 1.In Dagang oilfield area of Tianjin,children's overweight and obesity ratio are 15.00% and 11.97% respectively,and overweight-obesity ratio covers 26.97%.There are 1260 boys overweight,holding 67.31% of the total,and 612 girls overweight,covering the rest 32.69%;there are 999 boys obesity,accounting for 66.91% of the total,and 494 girls obesity,taking the rest 33.09% proportion.From the above data,we can see that overweight and obesity ratio in male children and adolescents are obviously higher than that in female;and with the growing of age,the obesity ratio in both male and female children.2.As the 876 students' blood tests show,the uric acid,triglyceride,and total cholesterol of 118 overweight group objects (male: female=80:38)are obviously higher than those of 643 normal group objects(male: female=317:326)(P<0.05),while the high-density lipoprotein of overweight group is obviously lower than that of normal group.The uric acid,triglyceride,and total cholesterol of 115 obesity group objects(male: female=81:34)are obviously higher than those of 643 normal group objects(male: female=317:326)(P<0.05),while the high-density lipoprotein of obesity group is obviously lower than that of normal group,and by the same time the uric acid of obesity group is obviously higher than that of overweight group(P<0.05).The fasting blood glucoses in three groups have no statistic significance(P=0.338),but with the increase of children's BMI,the crisis of insulin resistance also increases(P<0.05).3.The LEP level of overweight and obesity group are both higher than that of normal group,and obesity group is greater than overweight group(P<0.05).The APN level of obesity group is lower than that of normal group(P<0.05),but has no evident difference with overweight group,and there is no obvious difference between overweight group and normal group(p>0.05).The FGF-21 level of obesity group is higher than those of overweight and normal group(P<0.05),while there is no evident difference between overweight group and normal group(p>0.05);Pearson Correlation Analysis indicates that HOMA-IR is positively correlated with LEP and FGF-21(r=0.563?0.301;P<0.05),and negatively correlated with APN(r=-0.376;P<0.05).Conclusion: 1.Dagang oilfield area of Tianjin is undergoing children and adolescents overweight and obesity epidemic period;2.Children and adolescents obesity is not only a kind of independent disease,but also a kind of hazard which may increase the risk of diseases in adult stage,such as hyperuricemia,hyperlipidemia,type II diabetes,atherosclerosis and metabolic syndrome,etc.;3.Children and adolescents obesity can lead to insulin resistance,which may have relation to adipose tissue disorderly secreting fat inflammatory factors.Testing serum LEP,APN and FGF-21 can provide more accurate and specific references for the prognosis of insulin resistance from the aspect of function disorder and abnormal distribution of adipose tissues.
