Glioma is a kind of tumor originated from neural ectoderm interstitial cell,which accounts for 40%~50% of all primary brain tumors.Glioma is the most common intracranial malignant tumor.According to the pathology,glioma can be divided into astrocytoma,medulloblastoma,glioblastoma and ependymoma,and so on.Glioma is the most common primary intracranial tumor,with a high incidence of recurrence rate,high mortality rate and low curate and often appears to be not sensitive to radiotherapy or chemotherapy resistance in clinical treatment,leading to poor clinical treatment.So glioma is one of the most intractable diseases in Department of Neurosurgery.However,the specific pathogenesis of glioma is not yet clear.Epidemiological studies have confirmed that the electromagnetic radiation may increase the risk of glioma ailing important factors.In addition to environmental factors,genetic susceptibility genes play a role in the pathogenesis of glioma.IL-18 is capable of inducing interferon gamma(interferon-gamma,IFN gamma)model of cytokine production.Human IL-18 gene is located on chromosome eleventh(11q22.2-q22.3),which is expressed in many kinds of cells.IL-18 has a wide range of immune regulation characteristics,innate immunity in the body by IFN-gamma and CD134 pathway and acquired immune regulation in the host immune defense,plays an important role in the pathogenesis of tumor.However,there is little research on the association between IL-18 and glioma at home and abroad.Therefore,this study focuses on the relationship between serum IL-18 level and IL-18 gene polymorphisms and glioma.The aim of the study was to investigate the relationship between plasma IL-18 levels and glioma individuals,and research the association of IL-18 polymorphisms with the susceptibility and outcomes of glioma.Part 1 Association between serum IL-18 level and gliomaObjection: The level of serum IL-18 was detected in all individuals and investigated the relationship between the level of serum glioma and glioma.Methods: Enzyme linked immunosorbent assay was used to detect the concentration of serum IL-18 in 571 patients,and analyzed the relationship between serum IL-18 level and glioma.Result:A total of 182 patients with glioma and 389 healthy controls were included in this study.In the case group,there were 99 males(54.4%)and 83 women(45.6%).The age of the patients was 46.34±16.72 years old.In the healthy control group,the male was 200(51.4%)and female was 189(48.6%).The age was 45.56±13.76.There was no significant difference in gender and age distribution between the two groups(P = 0.58 and 0.28,respectively).In the case group,according to the histological classification of WHO glioma grade,I and were divided into low groups,71 cases ?(39.01%),grade III and IV were divided into advanced group,a total of 111 cases(60.99%).The serum IL-18 level in the case group was 477.85±110.34 pg/m L,and the serum IL-18 level in the healthy control group was 110.87±49.95 pg/m L,the serum IL-18 level was significantly different between the two groups(P < 0.01).The serum IL-18 level in the low level group was 456.61±106.95 pg/m L,the serum IL-18 level in the advanced group was 511.07±107.27 pg/m L,and the difference of serum IL-18 level between the two groups was statistically significant(P < 0.01).Conclusion: The level of serum IL-18 is associated with glioma,which has some clinical significance for the development and prognosis of glioma.Part 2 Relationship between IL-18 gene polymorphism and susceptibility to gliomaObjection: To investigate the relationship between IL-18 gene polymorphism and susceptibility to glioma,and to provide scientific basis for individualized treatment of glioma.Methods: Taqman-MGB technique was used to detect the polymorphism of IL-18 gene rs1946519,rs187238 and rs549908 in 182 glioma patients and 389 healthy controls.Result:The study of general characteristics with the first part 1;Logistic regression analyses showed that no significant difference was found between rs1946519,rs187238 polymorphisms and glioma,and the rs549908 heterozygous AC genotype distribution in the case group is significantly lower than in healthy controls(adjusted OR = 0.64,CI = 95% 0.42-0.98),suggesting that the risk of AC genotype can significantly reduce glioma,the dominant model shows that compared with the wild homozygous AA,the risk of rs549908 genotype AC+CC also significantly reduce glioma(adjusted OR = 0.59,CI = 95%;0.39-0.89);Haplotype analysis showed that,compared with the TGA haplotype,carrying GGC haplotype individuals decreases the risk of glioma(adjusted OR = 0.51,95%CI = 0.26-0.99);logistic regression analysis was performed to analyze the relationship between IL-18 gene SNPs polymorphisms and different grade glioma,the result showed no association between IL-18 gene SNPs polymorphisms and different levels of glioma(P > 0.05).Variance analysis was used to analyze the relationship between the genotype distribution of IL-18 gene rs549908 and serum IL-18 level.The results showed that in the case group and the healthy control group,there was significant difference in the serum IL-18 level between the different locus of rs549908 locus(F = 163.42,P < 0.01)Conclusion: IL-18 gene polymorphisms is associated with susceptibility to glioma,and may affect the occurrence and development of glioma by affecting the level of IL-18.
