Objective To study the MRI sequences optimized combination of cartilage damage,and evaluate its value at the time of acute knee cartilage injury.Materials and Methods Collet10 healthy volunteers and 40 patients with acute traumatic knee.Using MR360 1.5T MRI of GE to conventional scan(T1WI,T2 WI,PDWI),and FS-3D-SPGR,Fiesta-c,MEGRE sequence,each of joint observation of six surface(ie after patella cartilage,femoral block,within the femoral condyle,tibia,lateral plateau),where 10 cases of 120 cartilage healthy volunteers as normal cartilage,measured its SNR and CNR with a variety of organizations around,and 40 patients with acute knee trauma were treated by clinical or arthroscopic surgery within two weeks.We see clinical or arthroscopic surgery as the gold standard to evaluate the accuracy of the above sequence at different levels of cartilage injury,the sensitivity,specificity and consistency.Result: A total of 120 articular cartilage surfaces of 10 healthy volunteers were statistically analyzed,focuses on unconventional sequence FS-3D-SPGR,Fiesta-c,MEGRE the SNR,which(1)Fiesta-c sequence with high cartilage SNR,and a statistically significant(P <0.05)and the rest of the sequence differences;SNR value(2)3D-FS-SPGR sequence is higher than the rest of the sequence in addition to Fiesta-c,but the differences between the conventional sequence of no statistical significance,(P> 0.05);(3)MEGRE sequences in this study,each group below the selected sequence,but the sequence between T2 WI was no significant difference(P> 0.05).2.Statistical analysis of each sequence display contrast to noise ratio cartilage and surrounding tissues,including Fiesta-c sequence shows good contrast cartilage / surrounding tissue.3,A total of 40 patients with a total of 240 preoperative images of the joints,according to cartilage injury levels and clinical surgery control,which(1)FS-3D-SPGR(fast)sequence was 94.29%,a specificity of 95.29%,Kappa value of 0.793;sensitivity(2)Fiesta-c sequence of 92.21%,a specificity of 98.16%,Kappa value of 0.896;sensitivity(3)MEGRE sequence was 90.54%,a specificity of 95.18%,Kappa value of 0.558.Conclusions: 1,Fiesta-c sequence regardless of the cartilage from the display or conformity with the clinical surgery,should significantly higher than the other sequences inthis study can be used as the preferred sequence of suspected acute traumatic knee cartilage injury;(2)FS-3D-SPGR(fast)sequence after parameter optimization,scan time was significantly shorter in this experiment has also been good results,optimizing the sequence can be used as a cartilage injury;3,MEGRE sequences in this study,Whether from cartilage display,or with the clinical outcome control,no desired effect;4,when acute knee trauma with cartilage damage,the proposed sequence combinations: Sagittal SE sequence T1 WI,FSE sequence T2 WI / fs;coronal PDWI / fs,SE sequence T1WI;three-dimensional fat-suppressed Fiesta-c.
