Objective To summarize the short-term effect of the hybrid surgery in the treatment of aortic arch lesions.Method We retrospectively analyzed the clinical data of 7 patients from January 2016 to December 2016 in the Department of Cardiovascular Surgery,Fujian Provincial Hospital,using the hybrid surgery of non-aortic replacement(AA-IA-LCCA-LSA By-Pass+EVR)for the treatment of aortic arch lesions,including aortic arch aneurysm and aortic dissection,involving aortic arch but not involving ascending aortic.There were 6 males and 1 females with the mean age of(63.57 ± 8.72)years,aged 53 years to75 years.Including aortic dissection(Stanford type B)in 2 cases which the tear were located in the anonymous artery opening area or its vicinity,Aortic arch aneurysm in 4cases,one of which was aortic arch pseudoaneurysm.And 5 cases complicated with blood hypertension,5 cases with pulmonary infection,6 cases with pleural effusion,1case with cerebral infarction.Another 1 case just undergoing surgery due to car accident.The bleeding,operation time,intraoperative DSA angiography,the length of staying hospital after operation,perioperative death and complication were observed.Follow-up from 1,3,6 and 12 months after operation,including late death,complications,and the CTA results.Result The operative success rate was 100%(7/7),the intraoperative bleeding was 457.14 ±83.81 ml,the operation time was 209.71 ± 25.18 min,and the length of staying hospital after operation was 17.43 ± 7.79 days.All patients were successfully completed the hybrid surgery of non-aortic replacement(AA-IA-LCCA-LSA By-Pass+EVR),there were no death and complications during the perioperative period.And the intraoperative angiography confirmed that the bypass of blood vessels and anastomosis were unobstructed,the stent morphology were well adhered,and showed no obvious leakage and displacement.7 patients were followed up for 1 to 12 months.All patients survived and returned to normal life.No complications such as stroke and stent leakage were found.In all 7 patients,CTA showed that the stent was well adhered and had no obvious leakage and displacement,each bypass blood vessels and anastomotic were unobstructed.Conclusion The application of hybrid surgery in the treatment of aortic arch lesions has reliable effect,few complications,quick recovery and good short-term result.
