Objective: To investigate the role of time from onset to initial treatment on the prognosis of sudden sensorineural hearing loss(SSNHL),aim at provide guidance for the treatment of SSNHL.Method: We collected 277 adult patients with SSNHL from January 2015 to September 2016,all of the patients were treated in the Department of Otolaryngology Head and Neck surgery of the First affiliated Hospital of Fujian Medical University,eliminating the patients with bilateral SSNHL,Meynier's disease,otitis media,and the patient who was less than 18 years or pregnant.192 patients were reviewed retrospectively finally.We used the Logistics regression method to adjust some confounding factors such as age,gender,initial degree of hearing loss,audiogram type,and analysised the effect of time from onset to initial treatment on the prognosis of sudden sensorineural hearing loss.Results: Taking the time from onset to initial treatment as continuous variable,Logistic regression analysis showed that the risk of no recovery increased with the increase of time from onset to initial treatment,after adjust the influence of age,gender,initial degree of hearing loss,audiogram type.According to the different time from onset to initial treatment in different patients,the time was divided into three groups,there were 124(64.58%)patients in the ?3 days group,51(26.56%)patients in the 3 to 7 days group and 17(8.85%)patients in the > 7 days group.Logistic regression analysis showed that the risk of no recovery increase 0.238 times in 3 to 7 days group(OR =1.238,95% CI(0.613,2.498),P =0.5516)compared with patients in the ?3 days group,and increase 3.804 times in > 7 days group(OR =4.804,95% CI = 1.324-17.433,P=0.0170),after adjust the influence of age,gender,initial degree of hearing loss,audiogram type.Conclusions:The longer of the time from onset to initial treatment in the patients of SSNHL,the therapeutic efficacy is worse.The therapeutic efficacy decreases significantly after 7 days from onset.
