The Diagnosis Value Of AFP-L3,AFP And VEGF In Elderly Primary Hepatic Cancer And Efficacy After TACE

Objective To investigate the diagnosis value of AFP-L3,AFP and VEGF in elderly primary hepatic carcinoma and efficacy after TACEMethods 42 cases of PHC patients(PHC group),30 cases of benign tumor of liver(benign tumor liver group)were selected.VEGFwas detected by Double antibody sandwich ELISA,AFP and AFP-L3 were detected with ECLIA,AFP-L3 was isolated with ACSC(lens culinaris agglutinin(LCA)-coupled spin column)Results The serum levels of AFP-L3,AFP and VEGF in different ages,genders,tumor sizes,hepatitis B virus carriers was not significantly different(P>0.05).The serum levels of AFP-L3,AFP and VEGF in hepatic cirrhosis group was significantly higher than no cirrhosis group(P<0.05).The serum levels of AFP-L3,AFP and VEGF in PHC group was significantly higher than benign tumor of liver group(P<0.05).The serum level of AFP was significantly lower than that of preoperative(P <0.01).The serum level of AFP was significantly lower than that of preoperative(P> 0.05).The serum level of AFP-L3 was lower than that of preoperative one week and one month after operation(P <0.01).The area of AFP?AFP-L3?AFP-L3% under the ROC curve in the differential diagnosis of PHC were 0.790?0.834?0.681.And the area of AFP-L3 is the largest.The sensitivity and specificity of AFP –L3were 82.1% and76.7%,The sensitivity and specificity of AFPwere 67.3%and 72.5%,The sensitivity and specificity of VEGF were59.65%and 80.1%.Compared with the single detection,the sensitivity and specificity of AFP + AFP-L3 were94.1% and 55.6%,the sensitivity and specificity of AFP + AFP-L3+VEGFwere97.6% and 47.6%.Conclusions The diagnostic value of AFP-L3 is better than AFP.The allied combination of serum AFP,AFP-L3 and AFP makes up for the insufficient clinical applications of individual serum makers.
