Objective:Intestinal cleaning is routine preparation for colonoscopy.This article preliminaryly studied the effect of intestinal cleaning on the clinical symptoms and intestinal microbiota in patients with D-IBS,putting forward a new idea on the role of intestinal cleaning in IBS.Methods:According to the inclusion and exclusion criteria,the data of 40 cases of colonoscopy patients,including 20 cases with D-IBS and 20 cases without obvious symptoms,were collected in Gastroenterology Department of Shanxi Provincial People's Hospital from August 2014 to March 2015.We scored and compared the symptoms before and after cleaning and 12 months later on the basis of the patients' statement.Then we took quantitative detection on the 11 kinds of dominant intestina bacterium before and after cleaning using real-time fluorescent quantitative PCR technology.After that,two independent sample t-test was used to compare the difference between groups,and correlation anlysis used the Spearman's.Results:After intestinal cleaning,patients with D-IBS get significant decrease in the score of symptoms,and obtain an obviously prolonged remission stage,and the phenomenon which frequent occurred before examination remarkably improved.Variation features of fecal flora: Compared with the control group,D.Pne significantly increased in D-IBS group before cleaning(P < 0.05)and the difference disappeared after cleaning.Moreover,Bac in D-IBS group was significantly reduced(P < 0.05)after cleaning.Variation features of descending colonic mucosal flora: Bac,B.pre and Sg in D-IBS group was significantly decreased than that in the control group after cleaning(P < 0.05).However,Bif,E colil,EPEC,F.Pra,Firm,Lac,Sal between two groups had no obvious difference in both feces and descending colonic mucosa(P > 0.05).Correlation analysis showed there is a positive correlation among lactobacillus,clostridiumin and feacallibacterium but a negative correlation between lactobacillus and bifidobacterium in colonic mucosa tissue.Salmonella and E coli in feces were positively correlated with their content in colonic mucosa tissue,respectively.Conclusion:Intestinal cleaning can immediately alleviate symptoms and achieve relatively long-term relief which may be related to the reconstruction of intestinal flora after cleaning and the decreased engraftment of fecal pathogenic bacteria.Intestinal cleaning is expected to serve as a new adjuvant therapy to relieve IBS symptoms effectively for a long time.
