Objective:Through the clinical comparison of different time given enteral nutrition in patients with gastric cancer,observation and analysis the index on intestinal mucosal barrier function,to explore the effect of early enteral nutrition on intestinal mucosal barrier in patients with gastric cancer.Methods:According to the inclusion criteria from June 2015 to October 2016 in the First Affiliated Hospital of Shanxi Medical University Department of gastrointestinal surgery underwent radical gastrectomy in 60 patients as the research object,randomly divided into observation group(early enteral nutrition group,n=30)and control group(non-early enteral nutrition group,n=30).Early enteral nutrition group received nutrition in 6 hours after operation,non-early enteral nutrition group received nutrition after 48 hours.Blood samples were observed the 1st day before operation and the 1,4,7 day after operation respectively to measure about response indicators(ET,DAO and D-lac)and observe the clinical results of the two groups,and record the postoperative infectious complications(incision infection,pulmonary infection,urinary tract infection,abdominal abscess).Results:Compared observation group with control group,all the preoperative detection indexes were not significantly different(P>0.05).The response indicators(ET,DAO,and D-lac)of the 1st day after operation were significantly higher than preoperative,and reached statistical significance(P<0.05);The indicators of the 4d and 7d have differentdegrees of decline,but still higher than the preoperative level,the difference was statistically significant(P<0.05);There was no significant difference between the two groups after operation for the 1st day(P>0.05).After the operation,the indexes of the 4 day and the 7 day in early nutrition group were lower than the non-early nutrition group,the difference was statistically significant(P<0.05);Compared with the non-early nutrition group,the incidence of postoperative complications was lower in early nutrition group,the difference was statistically significant(P<0.05).Conclusion:The indexs(DAO,D-Lac,ET)were significantly increased in patients with gastric cancer after operation,suggesting that the intestinal mucosal barrier function was impaired in the early stage after operation;Early enteral nutrition on intestinal mucosal barrier function in patients with gastric cancer were significantly better than the non-early enteral nutrition,early nutrition group had lesst infectious complications than non-early nutrition group,suggesting that early enteral nutrition can reduce endotoxin translocation,maintain intestinal mucosal integrity,improve the prognosis.
