Objective:The aim of this study was to investigate the correlation between peripheral blood lymphocyte subsets,serum complement C3 and systemic lupus erythematosus,to diagnose the accuracy of SLE severe activity events,and to provide evidence for the clinical diagnosis and treatment of systemic lupus erythematosus disease severe activity events.Methods:This subject collects 350 patients with systemic lupus erythematosus and 220 age-and sex-matched normal healthy controls as the study object.The clinical data were collected and retrospectively analyzed.The patients were divided into four groups according to the systemic lupus erythematosus disease activity index(SLEDAI)for the stationary group,mild activity group,moderate activity group,severe activity group.The serum complement C3,peripheral blood lymphocyte subsets(T cells,B cells,Th cells,Ts cells,NK cells)were selected from the study group.Compared to healthy control group,the patients with SLE were treated with statistical methods(t test,variance analysis,Pearson analysis)to analyze the correlation with SLE disease activity.(ROC)curves were taken from the serum complement C3,peripheral blood lymphocyte subsets(T cells,B cells,Th cells,Ts cells,NK cells)to investigate the diagnostic value of the patients had severe activity in SLE patients.Results:1.Compared with the healthy control group,the level of complement C3 in SLE patients was significantly decreased(P <0.01),and the levels of complement C3 in the resting stage,mild activity,moderate activity and severe activity group were different(P <0.01).The absolute value of peripheral blood lymphocytes in SLE patients was lower than that in healthy controls(P <0.01).The peripheral blood lymphocytes of the resting stage,mild activity,moderate activity and severe activity group were different in SLE patients(P <0.05).Compared with the healthy control group,the total T lymphocytes,total B lymphocytes,Th cells,Ts cells,Th / Ts in the peripheral blood lymphocyte subsets of the SLE patients were different(P <0.05)(P <0.01).The levels of total T lymphocytes,Th cells,Ts cells and NK cells in the resting stage,mild activity,moderate activity and severe activity group were also significantly lower in the SLE patients group(P <0.01),total B lymphocyte,Th /Ts ratio was not statistically significant.2.The negative correlation between the disease activity of SLE and the complement C3(r=-0.44,P<0.01),peripheral blood lymphocyte absolute value(r=-0.24,P<0.05),total T cell(r=-0.298,P<0.01),Th cell(r=-0.299,P<0.01),Ts cell(r=-0.209,P<0.05),NK cell(r=-0.333,P<0.01),Th / Ts(r=-0.24,P<0.05).3.To the occurrence of severe disease activity(SLEDAI?15 points)as the state variables,the receiver operating characteristic(ROC)curve was generated using the following laboratory variables as test variables: the area under the curve(AUC)of C3 complement was 0.711(P<0.01),and the cut-off value was 0.37 g/L.The sensitivity and specificity of C3 complement were 56.52% and 87.67%,respectively,the lymphocyte absolute value of AUC was 0.634(P <0.05),the cut-off value was 0.75 × 109/L,the sensitivity was 60.87% and the specificity was 68.57%;the total T cell showed a AUC of 0.714(P <0.01)and a cut-off value of 786 / uL,with a sensitivity of 69.6% and a specificity of 69.9%;the AUC of Th cell was 0.709(P <0.01),and the cutoff value was 448 cells/?L.The sensitivity and specificity of Th cell were 86.96% and 47.95%,respectively.The results showed that the AUC was 0.659(P <0.01),the cut-off value was 313/uL,the sensitivity and specificity of Ts cells were 60.87% and 77.78% respectively.NK cell results showed AUC of 0.645(P <0.05),the cut-off value of 54 / uL,the corresponding sensitivity of 65.22%,specificity of 70.83%;Th / Ts showed a AUC of 0.652(P <0.05)and a cutoff value of 0.58.The sensitivity and specificity of the Th / Ts ratio were 52.17% and 81.94% respectively.The AUC of all the laboratory indicators was 0.817,the specific sensitivity was 90.5% and the specificity was 53.7%.The AUC of the serum complement C3 level and the absolute value of the peripheral blood lymphocytes were 0.744,the specific sensitivity was 90.5% and the specificity was 41.8%.The AUC of the peripheral blood lymphocyte subsets(T cells,B cells,Th cells,Ts cells,NK cells)were 0.771,the corresponding sensitivity was 76.2%,and the specificity was 41.8%,The specificity was 65.7%.Conclusion:1.Compared with the normal control group,the patient group(quiescent,mild activity,moderate activity,severe activity)complement C3,peripheral blood lymphocyte absolute value,peripheral blood lymphocyte subpopulation of the indicators are different.2.The negative correlation between the disease activity of SLE and the complement C3,peripheral blood lymphocyte absolute value,total T cell,Th cell Ts cell,NK cell,Th / Ts.With the rise of SLEDAI score decreased,and SLE disease activity was negatively correlated.3.In the diagnosis of SLE severe activity(SLEDAI ? 15 points),the comprehensive laboratory indicators(serum complement C3,peripheral blood lymphocyte absolute and subgroup)diagnostic value is the largest,and a single laboratory indicators of total T cell diagnosis is the most valuable.
