Abstract/Summary:
Objective:To investigate the value of aquaporins imaging with multi b-values diffusion weighted magnetic resonance imaging in the grading diagnosis of cerebral astrocytoma.Methods:Fifty-two patients with cerebral astrocytoma confirmed by histopathology have been collected from the First Hospital of Shanxi Medical University from April 2014 to July2016.Based on the 2016 World Health Organization(WHO)classification of tumors of the central nervous system, all the cases were devided into two groups: the low-grade group(WHO I-II grade)included twenty-four cases and the high-grade group(WHO III-IV grade)included twenty-eight cases, and all of the cases were underwent the conventional MRI scanning, the conventional DWI scanning and the multi-b values diffusion weighted imaging scanning before the surgery.Acquire the AQP-ADC figure and the conventional ADC figure in the post-processing workstation, and measure the AQP-ADC values and the conventional ADC values in the tumoral solid part and the contralateral normal white matter in the same region of interest(ROI).Using independent samples t-test to compare the AQP-ADC values and the conventional ADC values between the low-grade group and high-grade group of the tumoral solid part and the contralateral normal white matter, and receiver operating characteristic(ROC)curve to compare the diagnostic performance of the AQP-ADC values and the conventional ADC values of the grading of the cerebral astrocytoma.Results:(1)The AQP-ADC values and the conventional ADC values in the contralateralnaomal white matter between the high-grade group and the low-grade group were no significant differences(P=0.224,P>0.05);(2)The AQP-ADC values in tumoral solid part of the high-grade group of the cerebral astrocytoma were higher than those of the low-grade group, and the conventional ADC values in tumoral solid part of the high-grade group were lower than those of low-grade group;the AQP-ADC values and the conventional ADC values in tumoral solid part of the cerebral astrocytoma had significant differences between the high-grade group and the low-grade group(P<0.05);(3)The area under the curve(AUC)of the AQP-ADC values and the conventional ADC values were 0.891,0.778 respectively;the cut-off values were 0.120,1.012respectively;the sensitivity and specificity were 92.3% and 74.1%,84.6% and 61.5%respectively.Conclusion:(1)AQP-ADC values in tumoral solid part of the cerebral astrocytoma could be used to identify the low grade cases from the high grade cases;(2)AQP-ADC values were better than the conventional ADC values in the grading diagnosis of the cerebral astrocytoma.
