Objective:CT perfusion parameters of non small cell lung cancer were used to infer the mutation of EGFR gene,so as to find individual and optimal treatment strategies for patients in early stage.Methods:From 2015.10 to 2017.3,collected in our hospital suspected "pulmonary mass" and done CT perfusion examination.Confirmed by the pathology of non small cell lung cancer and the EGFR gene mutation detection.We collected 30 effective cases.According to the EGFR gene in patients with mutations,30 cases of lung cancer were divided into EGFR group and EGFR group of wild mutation and 13 cases of mutations group(13/30,43%),of which 8 cases of male,female 5 cases and 17 cases of wild group(17/30,57%),male 11 cases,female 3 cases.Blood flow(BF),blood volume(BV),mean transit time(MTT),and surface permeability(PS)were calculated in patients with lung cancer.The BF,BV,MTT and PS were compared between the patients with the mutation and the wild group.The data using two independent sample t test statistical analysis and using the ROC curve to analyze the perfusion parameters to evaluate the prediction ability of discrimination mutant EGFR.To obtain the optimum threshold and the corresponding sensitivity and specificity.Results:1.In 30 cases of non-small cell lung cancer patients,EGFR mutations in 13 cases,accounting for 43% of the study,of which 9 cases of adenocarcinoma,squamous carcinoma in 4 cases;EGFR wild group in 17 cases,accounting for 57% of the study,in which 7 cases of adenocarcinoma,10 cases of squamous carcinoma.2.The BF(t=2.514,P=0.021),BV(t=0.467,P=0.646),PS(t=1.162,P=0.260),parameters of EGFR mutant group were higher than those of wild group.While MTT(t =-0.656,P = 0.520)was higher in the wild group.There was no significant difference between in BV,PS and MTT mutant group and wild group(P> 0.05).BF value was statistically significant(P <0.05).3.The ROC curve of the BF value of mutant group and wild group was analyzed.It can be concluded when the BF value of 46.747mlˇ100ml-1ˇmin-1 be judged that non-small cell lung cancer EGFR gene mutation.The sensitivity and specificity were 80%,72.7%.Conclusion:CT perfusion examination have important clinical value in the predicting mutations of EGFR in NSCLC patients:(1)The BF,BV,PS parameters of EGFR mutant group were higher than those of wild group,the reason is that EGFR gene mutation may lead to enhanced activity and tumor angiogenesis,blood flow speed.(2)Measurement of BF can be used to determine the genetic mutation in patients with NSCLC.
