Objective:Evaluation of endoscopic papillary large balloon dilation(eplbd)treatment of common bile duct stones(CBDS)safety and effectiveness.Methods:Between June 2013 and May 2015,a total of 82 patients underwent eplbd with or without est for the management of cbds in the First Hospital of Shanxi Medical University.According to the inclusion and exclusion criteria,est combined with eplbd group was 48 cases,eplbd group was 35 cases.The clinical data of two patients,the stone clearance rate,the rate of using mechanical lithotripsy and complications were compared.Results:Complete stone clearance rate of est combined with eplbd group and eplbd group were 46 cases(95.8%),33 cases(94.3%).The application of mechanical lithotripsy rate were 7 cases(14.7%),7 cases(17.1%).There were no significant differences.The median of two groups using the balloon is 15 mm,P = 0.548.The operation time(from successful bile duct cannulating to complete stone removal)were 35.92 + 7.67 min,32.66 + 6.02 min.The procedure time was shorter in the eplbd alone group significantly.The incidence of complications of the two groups were similar,for example,pancreatitis and hyperamylasemia,the difference was not statistically significant.Conclusion:Simple eplbd and eplbd combined with est is effective and safe for the treatment of common bile duct stones.EST prior to eplbd may be unnecessary.
