Efficacy Analysis Of Three Non-surgical Treatments For Chylous Leakage After Abdominal Surgery

Objective To observe the three non-surgical treatments in chylous leakage after abdominal surgery,and compare the efficacy to choose the best non-surgical treatment for the disease.Methods This study retrospectively reviewed patients who underwent abdominal surgery and developed chylous leakage,from June 2011 to June 2016,at the Second Affiliated Hospital of Chongqing Medical University.There were originally 35 patients with chylous leakage complications after they underwent abdominal surgery.Five patients were excluded because they did not meet the inclusion criteria.Finally 30 patients were included and divided into 3 groups:LFD group[n=7,patients treated with low fat diet(LFD)],TPN group[n=10,patients treated with total parenteral nutrition(TPN)]and TPN+S group[n=13,patients treated with total parenteral nutrition and somatostatin(TPN+S)].The chylous leakage disappearance time,recovering oral intake time,drainaging tube placement time and hospital stay time after chylous leakage were compared among the three groups.Results The chylous leakage disappearance time,recovering oral intake time,drainaging tube placement time and hospital stay time were(24.29±5.59)d,(26.43±4.93)d,(29.00±4.62)d,(32.57±4.86)d in LFD group,(17.10±1.20)d,(20.20±1.03)d,(23.30±1.64)d,(26.60±2.17)d in TPN group,(15.69±1.03)d,(18.77±1.01)d,(21.54±1.45)d,(24.38±1.56)d in TPN+S group,respectively.There were significant differences in the chylous leakage disappearance time,recovering oral intake time,drainaging tube placement time and hospital stay time after chylous leakage among the three groups(P<0.05).The efficacy of chylous leakage was best in TPN+S group,better in TPN group,and worst in LFD group.Conclusions Chylous leakage is a rare complication in abdominal surgery,almostly can be cured by conservative treatment in up to 100%.The total parenteral nutrition and somatostatin should be the standard treatment.
