Backgroud:Breast cancer has become the first malignant tumor in women.Compared with western countuies,breast cancer onset age is early about 10 years in our country.it's focused on the 45 to 55 years old.Although nowadays breast cancer patients tend to younger,But with the progress of population aging,eldly breast cancer has increased.At present,the definitions for the age of elderly breast cancer and there are many controversy in the treatment programs,and for the treatment of axillary lymph nodes also have some differences opinion.The current research on whether the axillary lymph node dissection is helpful to improve the overall survival,reduce the rate of local recurrence and distant metastasis is still no unified conclusion.Objective:In eldly breast cancer,compared with no axillary lymph node dissection,whether the axillary lymph node dissection(ALND)can improve the prognosis of patients.To provide basis for select the clinical surgery.Methods: Research published literature on the effects of axillary lymph node dissection and unclean on the prognosis of elderly breast cancer,and make systematic evaluation by use Meta analysis,to assemble the effect of axillary lymph node dissection on overall survival,local recurrence and distant metastasis in eldly breast cancer which have breast surgery.Result:Seven studies were enrolled which include 6943 patient,5272 received and 1671 did not receive axillary dissection axillary lymph node dissection(ALND),most of the included cases were clinically diagnosed with early,axillary lymph node-negative.Meta analysis show that ALND could decrease the rate of axillary recurrence in eldly breast cancer,the RR was 0.12(95% CI 0.03-0.43,P=0.001),but no significant differences were found in overall survival,breast cancer-special mortality,local recurrence and distant metastasis.Conclusion: For early elderly breast cancer who can tolerate surgery,Axillary lymph node dissection does not improve patient overall survival and reduce tumor-specific mortality.Therefore,to avoid axillary lymph node dissection is a reasonable choice of surgical options.There are still few articles on axillary lymph node dissection effect the prognosis of elderly breast cancer,it's need more and large clinical studies confirmed.
