MRI And X-ray Feature Of The Cuboid Bone In Diabetic Foot Patients

ObjectiveTo analyze the clinical and radiological data of diabetic foot(DF)and non-DF retrospectively and investigate the MRI and X-ray characteristics of cuboid bone of two groups.MethodsClinical and radiological data of 124 patients with different types of foot lesions in our hospital from August 2006 to October 2015 were reviewed(58 males,66 females),include 54 cases in DF group,70 cases in non-DF.Marrow edema and fracture of cuboid bone in MRI and fracture line in X-ray were observed of DF group and non-DF group,respectively.Using Chi square test to compare 1.The difference of incidence of cuboid signal change between insulin-dependent group and non-insulin-dependent group;2.The signals of cuboid bone changes,simple marrow edema,marrow edema with fractures and the relationship between cuboid bone and the surrounding in DF group and non-DF group in MRI and X-ray were compared.ResultsThere were 54 patients with DF,in which 49 patients with insulin-dependent,4 patients with non-insulin-dependent and 1 with no history of drug therapy.There was no statistical significance of cuboid signal changes between insulin-dependent group and non-insulin-dependent group [44.9%(22/49)vs50.00%(2/4),(P=0.22)].There was statistical significance between groups in signals of cuboid bone changes,simple marrow edema,marrow edema with fractures and the relationship between cuboid bone and the surrounding(all P< 0.05=.In DF group,18 patients(18/54,33.33%)were with simple marrow edema performed as iso-or low signal intensity on T1 WI,hyperintense on T2 WI,and no significant enhancement were observed.In non-DF group,8 patients(8/70,11.43%)suffered from simple marrow edema.In DF group,6 patients(6/54,11.11%)suffered from marrow edema with fractures performed as thready and fuzzy low signal intensity on T1 WI,only 1 patient was observed in X-ray;In non-DF group,only 1 patient(1/70,1.43%)suffered from marrow edema with fractures,no one was observed in X-ray.In DF group,1 patient suffered form osteomyelitis in cuboid bone,3 suffered form accumulating fluid in articulatory antrum around cuboid bone,2 suffered form Charcot's arthropathy;only 1 patient suffered form accumulating fluid in articulatory antrum in non-DF patients.ConclusionThe cuboid bone is more vulnerable to marrow edema and fractures in diabetic patients compared to non-diabetic patients,combined with the MRI imaging features is helpful to diagnose the cuboid bone changes in DF patients.
