Clinical Research On Hemisphericdisconnection For Intractable Epilepsy

Objective To discuss operation method and the curative effect of hemisphericdisconnection for intractable epilepsy.Methods Retrospective collection the clinical data of patients from August 2014 to January 2016 in our hospital,including the diseases history,clinical symptoms,imaging data and the result of VEEG.All patients were treated by hemisphericdisconnection.Step by step off of the ipsilateral hemisphere: first through lateral fissure resection through temporal hippocampus and amygdala;angle along the triangle to the frontal cortex segmented fistula,segmented off frontal,parietal lobe and occipital lobe and corpus callosum;finally through lateral fissure resection of insular cortex.Results The procedure was successfully performed in 4 patients.All 4 patients were followed up for 8-20 months(mean,13 months).The results showed that the operation trauma was small,the complications were less,and there was no complication in the patients with postoperative dysfunction of the contralateral limb.Epilepsy was effectively controlled,3 cases of Engel grade I,Engel grade II in 1 cases.And postoperative recovery,neuropsychological,mental behavior improved significantly.Conclusion Hemisphericdisconnection is a kind of hemisphere lesions caused by intractable epilepsy patients with trauma,reliable treatment effect.
